Social Worker

Location: Orange County, CA 92868
Job Type: Contract
Duration: Up to 6 months
Schedule: Monday–Friday, 8:00 AM–4:30 PM
Pay Rate: $32–$45/hour
Work Arrangement: Full Office

Job Overview

We are seeking a Social Worker to provide care management, psychosocial support, counseling, advocacy, and care coordination services for elderly participants. This role works closely with an interdisciplinary team to assess participant needs, develop care plans, coordinate services, and connect participants and families with appropriate community resources.

The position may require home visits and travel throughout Orange County.

Key Responsibilities

Participate in initial, six-month, and annual assessments and care planning as part of an interdisciplinary team.
Assess participants' psychosocial needs and develop appropriate care plans and interventions.
Document participant changes and services accurately in the medical record.
Communicate participant status, needs, and care plans effectively with interdisciplinary team members.
Assess and respond appropriately to high-risk situations, including suicidal/homicidal ideation and abuse.
Develop and facilitate group counseling and participant support group activities.
Provide supportive counseling related to anxiety, depression, behavioral concerns, coping mechanisms, cognition, and other psychosocial needs.
Refer participants and families to appropriate community agencies and resources.
Serve as an advocate and liaison between participants, families, community organizations, and care teams.
Assist participants with advance directives, facility placement, funeral planning, and related arrangements.
Assist with identifying third-party and financial resources for services when needed.
Support discharge planning for participants transitioning from services.
Coordinate transportation, appointments, family communication, and warm handoffs to community partners.
Support enrollment activities and help identify and address service gaps.
Participate in case conferences, staff meetings, in-services, and department improvement initiatives.
Collaborate with Quality Improvement teams regarding grievances, appeals, and participant advocacy.
Perform other duties and projects as assigned.

Required Qualifications

Master’s degree in Social Work (MSW) from an accredited college or university.
2+ years of experience in a health-related field, or an equivalent combination of education and experience.
1+ year of experience working with the elderly population, with relevant experience within the last 3 years.
CPR and First Aid certification required or must be obtained within 6 months of hire.
Valid driver’s license, acceptable driving record, current auto insurance, and reliable transportation or another approved means of transportation for required community travel.
Strong communication, organization, analytical, and problem-solving skills.
Ability to work independently and exercise sound judgment.
Ability to manage multiple priorities in a fast-paced environment.
Proficiency with Microsoft Office applications, including Word, Outlook, Excel, and PowerPoint.

Preferred Qualifications

1+ year of case management experience working with the elderly population.
Bilingual skills in English and one of the following languages: Arabic, Farsi, Chinese, Korean, Russian, Spanish, or Vietnamese.
Experience working with interdisciplinary care teams.
Experience with community resources, care coordination, and participant advocacy.

Work Environment

Primarily an indoor/office-based position.
May involve travel to participants' homes and community locations throughout Orange County.
Position may involve working in clinical and community environments.
Some lifting or moving of objects, patients, or equipment weighing 10–25 pounds may be required.
Schedule may occasionally vary based on operational needs.

Pay: $32.00 - $45.00 per hour
