Our Church of Christ Care Community Family is growing, and we’d love to meet you!

We are currently recruiting for a full-time Social Worker who performs elected and delegated services for the residents of the nursing home and senior housing complex. Organizes and plans the Social Service Program for residents. The program is based on types of residents cared for and community facilities and agencies that can provide social, economic, psychological and medical assistance.
Qualifications

Minimum of a bachelor’s degree in social work or a bachelor’s degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology.
One year of supervised social work experience in a health care setting working directly with individuals.
Good physical health. Evidence of stable emotional and psychological adjustments.
Extroverted personality, capable of warm, friendly communication.
Must display a genuine interest in the elderly. Must be patient, kind, understanding, and capable of making good judgments and decisions relative to the spiritual, physical, and social environment of the residents.
Capable of organizing, planning, and implementing spiritual and social needs relative to the unique needs of the elderly.
Must relate effectively with residents, their families, fellow employees, and administrator.
The Social Worker shall have a minimum of a Bachelor's Degree in Social Work from an accredited school, a current Michigan Social Work license and one year full‑time experience in a health care setting. If the Designated Social Worker does not meet the above requirements, Church of Christ Care Center will have on contract a consultant with the above qualifications who has regularly scheduled consultation training and orientation with the Social Worker Designee.
Essential Job Functions
To identify the social and emotional needs of each resident and provide services to meet them at admission, during treatment and care in the facility, and in planning for his/her discharge
Upon admission, pertinent information is obtained, by use of a Social History, from the resident, family, referral sources and medical staff. The Social History is used by the Social Worker Designee and the facility's professional staff, as an aid in formulating a plan of care with both short term and long-term goals.
Maintains individual social service records in accordance with the facility standards and enters pertinent social factors on the resident's record.
Makes supportive visits to the resident and performs needed services either directly, or by referral to the facility's professional staff or outside resources.
Maintains contact with family members, involving them, as much a possible, in the resident's total care.
Collaborates with individuals and groups within and outside the facility in development of supportive services for residents in line with their interest and needs.
Collects information about community health and social services and keeps up‑to‑date resource file or handles such services directly.
Participates in the planning for the resident's return home or transfer to another facility.

Attends in‑service sessions and seminars recommended by the Administrator.

Assist with admissions when needed.
