Job Overview

We are seeking an experienced and dynamic Lead Care Manager to oversee care coordination and case management within our healthcare facility. The ideal candidate will possess extensive clinical expertise, including critical care, hospital, and emergency medicine experience, combined with strong leadership skills. As a Lead Care Manager, you will guide a team of care professionals to ensure seamless patient transitions, effective utilization review, and high-quality service delivery across various medical settings.

Responsibilities:

Lead and coordinate care management activities for a diverse patient population, ensuring comprehensive and continuous care.
Develop, implement, and monitor individualized care plans based on thorough patient assessments and clinical guidelines.
Facilitate communication and collaboration among healthcare providers, patients, families, and community resources to optimize care delivery.
Supervise and mentor care management staff, providing guidance and support to enhance team performance and professional development.
Track patient outcomes and quality metrics, using data to inform care improvements and report to leadership and stakeholders.

Minimum Qualifications:

Bachelor’s degree in Nursing, Social Work, Healthcare Administration, or a related field.
Minimum of 5 years of experience in care management or case management within a healthcare setting.
Strong knowledge of care coordination principles, chronic disease management, and healthcare systems.
Excellent communication and interpersonal skills to effectively engage with patients, families, and multidisciplinary teams.
Proficiency in electronic health records (EHR) systems and care management software.

Preferred Qualifications:

Master’s degree in Nursing, Social Work, Public Health, or Healthcare Administration.
Certification in Case Management (CCM) or Certified Care Manager (CCM) credential.
Experience leading or supervising care management teams.
Familiarity with population health management and quality improvement methodologies.
Knowledge of community resources and social services relevant to patient care.
