Merrimack Valley Assistance Program (MVAP), founded in 1995, is New Hampshire’s largest HIV/AIDS Service Organization and a registered Syringe Services Program (SSP). As a 501(c)(3) nonprofit and state-licensed case management agency, MVAP offers comprehensive services, including case management, supportive services, housing assistance, and referrals to individuals living with HIV/AIDS.

Who We Are Looking For:
Merrimack Valley Assistance Program (MVAP) is seeking a compassionate, outgoing, resourceful, and community-focused Outreach Coordinator to join our team in Manchester, NH. This position will lead and expand MVAP’s outreach efforts both in the community and online, with a focus on reaching individuals who may be disconnected from traditional healthcare systems, including people experiencing housing instability, substance use disorders, mental health challenges, and other barriers to care.
The Outreach Coordinator will help increase awareness of MVAP’s prevention, testing, harm reduction, and supportive services while building trusting relationships with individuals and communities. The position will spend time both in the office and out in the community, providing harm reduction and overdose prevention services, HIV, Hepatitis C, and Syphilis rapid testing, health education, and referrals and connections to medical care, housing, behavioral health services, and other essential supports. The Outreach Coordinator will also help strengthen MVAP’s digital outreach by creating social media content that promotes available services, testing opportunities, harm reduction resources, outreach activities, and community events.
This is a hands-on outreach position. The Outreach Coordinator will conduct outreach in settings such as shelters, encampments, recovery centers, healthcare facilities, community organizations, public spaces, and other locations where individuals may experience barriers to care. The position will also oversee the day-to-day operation of MVAP’s Exchange Program and work closely with MVAP Case Managers and community partners to ensure individuals reached through outreach are successfully connected to care and services.
Most outreach activities will take place within the City of Manchester, with travel to other communities throughout New Hampshire as needed.
What You'll Be Doing:
As the Outreach Coordinator, you will help expand MVAP’s presence in the community and develop trusting relationships with individuals who may not otherwise access traditional healthcare or social service systems.
Your work will include:
- Conducting community-based outreach at shelters, encampments, recovery centers, community organizations, public spaces, healthcare settings, and other locations.
- Engaging individuals who are at risk for overdose, HIV, Hepatitis C, and other health disparities or who have fallen out of medical care.
- Coordinating and overseeing the day-to-day operation of MVAP’s Exchange Program, including harm reduction supply distribution and safe syringe disposal.
- Conducting HIV, Hepatitis C, Syphilis, and other approved point-of-care testing.
- Providing overdose prevention education and distributing naloxone and other harm reduction supplies.
- Create engaging social media content that promotes MVAP’s outreach efforts, testing services, harm reduction and overdose prevention resources, community events, and other programs and services.
- Providing education related to safer use practices, sexual health, HIV/HCV/STI prevention, and available community resources.
- Connecting individuals to medical care, behavioral health services, recovery resources, housing, and other supportive services.
- Providing warm hand-offs to MVAP Case Managers and community providers to help individuals successfully engage or re-engage in HIV care.
- Maintaining contact with individuals following outreach encounters when appropriate to help reduce barriers to continued care.
- Building and maintaining relationships with healthcare providers, shelters, recovery organizations, public health agencies, municipalities, emergency responders, law enforcement, and other community partners.
- Representing MVAP at outreach events, health fairs, coalition meetings, trainings, and other community activities.
- Tracking outreach activities, testing, referrals, supplies, and program outcomes in accordance with agency and grant requirements.
- Maintaining outreach supplies, testing materials, harm reduction supplies, and program inventory.
- Supporting MVAP’s food pantry operations and related outreach efforts, including picking up pantry orders, organizing and stocking inventory, and helping ensure food resources are available to eligible individuals and during outreach activities.
- Helping identify new outreach opportunities and strategies to strengthen MVAP’s reach throughout the community.
- Performing additional duties as assigned by the Executive Director.
Compensation & Schedule:
- Salary: $46,000
- Schedule: Monday - Thursday, 8:00 AM - 5:00 PM & Friday, 8:00 AM - 11:30 AM (37.5 hours/week). Because effective outreach does not always occur during traditional business hours, this position may include occasional evening and weekend outreach activities, community events, trainings, or other program-related activities as needed.

Important Funding Information: This is a grant-funded position currently funded through July 1, 2028. Continued employment beyond this date is contingent upon MVAP securing renewal or replacement funding. While MVAP actively pursues additional funding opportunities, continued funding and employment beyond July 1, 2028 cannot be guaranteed.
Qualifications & Requirements:
We recognize that effective outreach professionals come from many different backgrounds. Individuals with lived or living experience related to substance use, recovery, homelessness, HIV, Hepatitis C, or other barriers to healthcare are strongly encouraged to apply.
- Education and/or experience in public health, community health, social work, human services, behavioral health, or a related field.
- Experience in community outreach, public health, harm reduction, prevention services, behavioral health, or human services.
- Experience working directly with individuals experiencing homelessness and housing instability, substance use disorders, behavioral health conditions, or other barriers to care.
- Experience conducting HIV, Hepatitis C, STI, or other point-of-care testing.
- Knowledge of HIV/AIDS, Hepatitis C, sexually transmitted infections, PrEP/PEP, overdose prevention, substance use disorders, and harm reduction principles and practices.
- Experience with harm reduction services, syringe service programs, overdose prevention, naloxone distribution, or other community-based prevention services.
- Knowledge and understanding of trauma-informed and person-centered approaches to outreach and service delivery.
- Strong interpersonal and engagement skills, with the ability to build trust and rapport with individuals who may have experienced stigma, trauma, or significant barriers to traditional systems of care.
- Strong communication skills and comfort discussing sensitive topics, including substance use, sexual health, HIV/STIs, housing instability, behavioral health, and other health and social needs.
- Experience providing health education and prevention information to individuals, groups, or community partners.
- Experience building relationships and collaborating with healthcare providers, community organizations, social service agencies, and other partners.
- Knowledge of community resources, including healthcare providers, recovery services, housing programs, shelters, food resources, behavioral health services, and other social services.
- Experience working with diverse communities, including LGBTQ+, immigrant, refugee, undocumented, and BIPOC communities.
- Strong resourcefulness and problem-solving skills, with the ability to identify barriers and connect individuals with appropriate services and resources.
- Ability to work independently in community settings, exercise sound judgment, adapt to changing circumstances, and follow agency safety policies.
- Strong organizational and time-management skills, including the ability to coordinate outreach activities, maintain supplies and inventory, track program data, and complete required documentation accurately and timely.
- Ability to work collaboratively with MVAP staff, healthcare providers, community organizations, and other partners as part of a multidisciplinary service network.
- Proficiency with Microsoft Word, Excel, Outlook, and the ability to learn new databases, applications, and systems.
- Valid driver’s license, reliable transportation, and current automobile insurance with 100/300 liability coverage. Proof is required annually.
- Ability and willingness to travel regularly to community outreach locations, primarily within the City of Manchester and to other areas of New Hampshire as needed.
- Ability and willingness to occasionally work evenings and weekends based on outreach, community event, and program needs.
