Serve only the Eastern Panhandle — no long commutes.
Assigned territory for efficient scheduling and predictable travel.

What You’ll Do

Provide high‑quality physical therapy services in home health settings.
Assess functional abilities and help develop individualized plans of care.
Document accurately and maintain confidentiality.
Offer education, consultation, and coordination with families and care teams.
Participate in performance improvement activities and required meetings.
Uphold PHHI policies, professional standards, and the WV State Practice Act.

What We’re Looking For

Current West Virginia Physical Therapist license (in good standing).
Current CPR certification.
Experience as a Physical Therapist (home health experience a plus).
Strong critical thinking, judgment, and communication skills.
Ability to work well with patients, families, and team members.
Meets all federal program participation requirements.
Experience with discharge planning and developing comprehensive care plans for diverse patient populations.
Familiarity with EMR systems for documentation and patient management
Excellent communication skills to effectively educate patients and coordinate with healthcare teams
