Job Overview
We are seeking a dedicated and energetic Home Care Registered Nurse (RN) to join our compassionate team serving clients in Oakland County. This vital role involves providing high-quality, personalized nursing care in the comfort of patients’ homes, promoting independence, safety, and well-being. As a Home Care RN, you will be instrumental in delivering comprehensive assessments, developing individualized care plans, and ensuring optimal health outcomes for diverse patient populations. Your expertise will help empower clients to live healthier lives while supporting their families and caregivers.

Duties

Conduct thorough patient assessments, including vital signs, physical examinations, and detailed health histories to determine individual needs.
Develop and implement personalized care plans aligned with medical orders and patient goals, ensuring adherence to all healthcare standards.
Administer medications via injections, IV infusions, and other methods while monitoring for adverse reactions or complications.
Perform advanced procedures such as wound suturing, catheterization, ostomy care, and sterile dressing changes following aseptic techniques.
Manage complex cases involving chronic conditions like dementia, Alzheimer's disease, memory care, or long-term disabilities with tailored interventions.
Utilize electronic health record (EHR) systems to document patient interactions accurately and maintain comprehensive medical records.
Provide patient education on medication management, disease processes, nutrition, and self-care strategies to promote health literacy.
Collaborate with multidisciplinary teams including physicians specializing in ophthalmology, cardiology, critical care, or hospice & palliative medicine to coordinate seamless care transitions.
Support patients with acute pain management needs or those requiring specialized services such as dialysis or ventilator support in home settings.
Ensure compliance with HIPAA regulations and maintain confidentiality of all patient information throughout every interaction.

Qualifications

Valid RN license in the state of practice with current CPR/BLS certification.
Extensive nursing experience in diverse settings including hospital medicine, ICU, emergency room, nursing homes, assisted living facilities, or home care environments.
Proficiency with EMR/EHR systems such as Epic or Cerner; experience with ICD coding (ICD-9/ICD-10) is preferred.
Strong clinical skills in IV insertion, medication administration (including injections), sterile technique, phlebotomy, and specimen collection/processing.
Knowledge of complex medical procedures including tube feeding management and ventilator support is highly desirable.
Critical care experience such as Level I or Level II trauma center work or ICU experience is advantageous.
Ability to work independently while managing multiple cases involving behavioral health, geriatrics, pediatrics (including neonatal ventilator), or individuals with disabilities.
Excellent assessment skills combined with physiology knowledge to evaluate patient status accurately and respond promptly to emergencies like acute pain episodes or trauma situations.
Familiarity with Workers' Compensation law and post-acute care protocols enhances your ability to deliver comprehensive home-based services.
