Full Job Description

AgeWell PACE Community Outreach Specialist

Job Summary

The position reports to the AgeWell PACE Executive Director. The primary responsibility of the Community Outreach Specialist is the on-going recruitment and enrollment of PACE participants into the program and the growth and maintenance of an optimal enrollee census. In addition, the Community Outreach Specialist is responsible for supporting all public awareness efforts of the program including the marketing of services to providers, referral sources and the community at large and all public relations efforts both locally and statewide.

Position Responsibilities

Community Engagement: Build and maintain relationships with local communities, senior centers, faith communities, and other relevant organizations to promote awareness of PACE services.
Outreach Events: Plan and coordinate outreach events, health fairs, program tours, open houses, workshops, and informational sessions to educate the community about PACE and its benefits.
Marketing Materials: Develop brochures, flyers, and presentations to effectively communicate the value proposition of PACE to diverse audiences.
Networking: Collaborate and build relationships with healthcare providers, social workers, case managers, and others to identify potential participants for the PACE program and facilitate referrals.
Educational Initiatives: Conduct training sessions and educational workshops for community members, caregivers, and healthcare professionals to increase understanding of PACE services and eligibility criteria.
Data Tracking: Maintain accurate records of outreach activities, participant demographics, and referral sources to evaluate the effectiveness of outreach efforts and inform future strategies.
Cultural Sensitivity: Demonstrate sensitivity to the cultural and linguistic diversity of the community served, ensuring that outreach materials and activities are inclusive and accessible to all.
Partnership Development: Identify opportunities for partnership and collaboration with local agencies, advocacy groups, and community leaders to support the mission of the PACE organization. Plans and manages all marketing resources within an approved budget.
Works closely with the Intake team to ensure an efficient enrollment process for new PACE participants. May provide enrollment assistance, including in participants homes as needed.

Establish and maintain effective interpersonal relationships with all levels of staff, other programs, agencies, members, providers, and the general public.
Communicate effectively, both verbally and in writing, with individuals from varying cultural backgrounds. Strong presentation skills are essential.
Prepare concise and clear reports.
Effectively utilize computer and appropriate software and interact as needed with AgeWell PACE Information Systems.
Foster and model an effective team environment, where each team member is held accountable for their independent work.
Meet a standardized set of competencies for the specific position description established by AgeWell PACE and approved by regulatory agencies before working independently.
Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.
Pass a criminal background which will verify that the candidate is: not excluded from participation in Medicare and Medicaid programs; has not been convicted of criminal offenses related to their involvement in Medicaid, Medicare, other health insurance or health care programs, or social services programs under title XX of the Act; and have not been convicted of physical, sexual, drug or alcohol abuse.

Knowledge of

Culture and needs of the socially and ethnically diverse population AgeWell PACE serves.
Principles and practices of health care service delivery, managed care, health care systems.
Effective HMO, Medi-Cal, Medicare, and senior care related marketing & outreach activities.
Community-based organizations, agencies, and community services.
Strategies for effective communication with diverse communities and the ability to develop contacts and generate referrals.

Experience & Education

Bilingual (English/Spanish)
Bachelor’s degree in marketing, communications, public relations or related field desirable; High School diploma required.
Minimum two years’ experience working in a customer service role; outreach or sales experience preferred.
Knowledge of and/or experience with HMO, Medi-Cal/Medicaid, Medicare, and senior care related sales and marketing experience, preferred.
Familiarity and/or experience with caring for older adults.

Certifications/Licenses Required

CA valid driver’s license and reliable transportation.
Proof of current vehicle insurance (if applicable).
Must take and pass the State of California DHCS PACE and Medi-Cal Marketing Certification exam within 60 days of hire.
Valid BLS certification in accordance with the American Heart Association guidelines required upon hire.

Physical Demands and Work Environment

The physical demands and work environment characteristics described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Physical demands: While performing duties of job, employee is required to move about the PACE center and actively engage with the interdisciplinary team and participants. Employee must be able to sit for extended periods of time, as well as work at the computer for long periods. Employee is required to use hands and fingers, especially for typing on the computer and using the mouse. Employee must be able to talk and hear, particularly for phone communications.

Work Environment: Busy Adult Day Health Center (ADHC) and clinic environment serving the frail elderly with medium to loud noise levels and controlled office temperatures.
