ABOUT HOME MATTERS CAREGIVING

Home Matters Caregiving is a premium non-medical home care provider expanding across the Napa Valley and North Bay region. We deliver a Nurse-Guided, Technology-Driven experience that sets us apart in a crowded market. Our clients trust us with their most vulnerable family members — and we take that responsibility seriously.

We are looking for a seasoned Home Care Community Liaison who knows this industry, understands the referral ecosystem across Napa, Sonoma, Solano, and Marin counties, and is ready to build something real.

REQUIREMENT: Prior Home Care Experience Is Mandatory Candidates must have a minimum of 2 years of direct experience in private duty / non-medical home care business development, sales, or marketing. Applications without verified home care experience will not be reviewed.

KEY RESPONSIBILITIES

Referral Network Development

Build, nurture, and grow a high-value referral network across hospitals, skilled nursing facilities, assisted living communities, discharge planners, geriatric care managers, and physician practices throughout Napa, Sonoma, Solano, and Marin counties

Field-Based Outreach

Complete 30–40+ strategic, face-to-face professional visits per week
Serve as the face of Home Matters in this market — presence matters

Relationship Management

Break into existing competitor accounts, convert referral partners, and build long-term relationships through consistent follow-up and trust

Strategic Territory Planning

Develop and execute a data-driven 90-day referral growth plan
Track activity, measure results, and adapt your approach to the multi-county dynamics of this territory

Family Intake Support

Assist with initial family consultations and help ensure confident, smooth starts of care

Digital & Reputation Support

Partner with the team on local Google Reviews outreach and community presence-building across the territory

WHO YOU ARE

2+ years in home care sales, marketing, or business development (private duty / non-medical — non-negotiable)
Deep familiarity with home care referral sources, payer dynamics, and care coordination
Proven track record of territory growth and consistent referral generation in a competitive market
Knowledge of the Napa / North Bay healthcare landscape — existing relationships with Queen of the Valley, Kaiser Vallejo, Sutter Solano, or regional ALCs a significant advantage
Self-starter — you make the calls, show up, and follow through without being managed
Empathetic and persuasive — you build trust quickly with healthcare professionals and families
Comfortable with CRM platforms (Salesforce, HubSpot, or similar) and Microsoft Office
Polished, professional, and reliable in every interaction
Willing to travel across a multi-county territory — valid CA driver's license required

COMPENSATION & BENEFITS

Base Salary: $65,000 – $78,000 annually, based on experience (paid bi-weekly)
Commission: Uncapped monthly performance incentives

Pay: $65,000.00 - $78,000.00 per year
