Job Announcement: Family Development Coach

Status: Full-Time, In Person
Reports To: Program Director

Annual Salary: $47,500 – $50,000, commensurate with education and experience

Schedule: Monday – Friday, 9:00am – 6:00pm

The Mosholu Montefiore Community Center (MMCC) is a vital community resource that changes the course of lives of Bronx and Manhattan residents to a degree not otherwise possible. Each year, we serve more than 40,000 preschoolers, school-age children, teens, adults, and senior citizens through our support, enrichment, education, and recreation programs, expanding their opportunities to live full, productive lives.

Position Summary:

MMCC is seeking a compassionate and motivated Family Development Coach (FDC) to provide case management services to individuals and families. The FDC works collaboratively with participants to identify goals, address barriers, connect them to community resources, and support their progress toward self-sufficiency. This position requires a strong commitment to empowering participants through a strengths-based, family-centered approach.

Responsibilities:

● Conduct participant assessments to identify strengths, needs, and goals.

● Develop individualized Family Development Plans and monitor participant progress.

● Provide ongoing case management, coaching, and support to help participants achieve personal, educational, and employment goals.

● Connect participants with community resources, benefits, and supportive services.

● Maintain regular contact with participants through meetings, phone calls, and follow-up services.

● Accurately document all participant interactions, referrals, and case notes in the appropriate data management system.

● Assist participants in overcoming barriers related to education, employment, housing, health, and financial stability.

● Collaborate with internal programs and external community partners to coordinate services.

● Maintain participant confidentiality and ensure compliance with program and agency policies.

● Participate in staff meetings, trainings, and other program activities.

● Perform other duties as assigned by the Program Director.

Qualifications:

● Bachelor's degree in Social Work, Human Services, Psychology, Sociology, or a related field required.

● Minimum of 2 years of experience in case management, social services, family support, or a related field.

● Strong knowledge of community resources and social service systems.

● Experience with participant databases and case documentation

● Knowledge of DYCD Participant Tracking Systems (PTS) is preferred.

● Excellent communication, organizational, and problem-solving skills.

● Ability to work effectively with individuals and families from diverse cultural and socioeconomic backgrounds.

● Proficient in Microsoft Office Suite (Word, Excel, Outlook, PowerPoint) and Google Workspace.

● Bilingual (Spanish/English) preferred but not required.
