Nurse Manager- Assisted Living

The Grand at Twin Lakes – Palatine, IL

Full-Time

Qualifications Required

Active Illinois Nursing License (LPN or RN)
Minimum 2 years of nursing experience
Current CPR Certification
Strong assessment and clinical decision-making skills
Excellent communication and leadership abilities

The Grand at Twin Lakes is seeking a compassionate, organized, and clinically experienced Wellness Care Coordinator to lead our Assisted Living program. This position serves as the clinical leader for our Assisted Living residents and works closely with the Executive Director, families, physicians, and care staff to ensure exceptional resident care, regulatory compliance, and a resident-centered environment.

ResponsibilitiesClinical Leadership

Oversee daily resident care operations within Assisted Living.
Provide clinical guidance and support to CNAs, caregivers, and medication aides.
Monitor resident health status and coordinate timely interventions for changes in condition.
Participate in care conferences and interdisciplinary team meetings.
Respond to resident emergencies and assist with clinical decision-making.

Resident Assessments & Care Planning

Complete pre-admission, admission, 30-day, annual, and significant change assessments.
Develop, implement, and update individualized service plans.
Coordinate care plan meetings with residents, families, and healthcare providers.
Ensure service plans accurately reflect resident needs and physician recommendations.
Monitor outcomes and revise plans as needed.

Medication Management

Oversee medication administration systems and procedures.
Conduct medication audits and observations.
Review physician orders for accuracy and completeness.
Follow up on medication errors, refusals, and adverse reactions.
Ensure compliance with medication storage, documentation, and controlled substance requirements.

Leadership & Staff Development

Assist with interviewing, hiring, onboarding, and training resident care staff.
Provide coaching, mentoring, and performance feedback.
Conduct competency evaluations and skills validations.
Participate in corrective action and disciplinary processes when necessary.
Coordinate ongoing education and in-service training programs.

Regulatory Compliance

Maintain compliance with Illinois Assisted Living regulations and company policies.
Participate in state survey preparation and follow-up activities.
Conduct routine audits of resident records, medication records, and clinical documentation.
Monitor infection prevention and control practices.
Assist with incident investigations and corrective action plans.

Resident & Family Support

Serve as a resource for residents and families regarding care-related concerns.
Communicate changes in resident condition to physicians, families, and appropriate team members.
Promote resident satisfaction and positive family relationships.
Assist in resolving care concerns and conflicts professionally and compassionately.

Quality Assurance

Participate in Quality Assurance and Performance Improvement (QAPI) initiatives.
Track clinical indicators including falls, infections, wounds, weight changes, and hospitalizations.
Support quality improvement projects and performance goals.
Ensure timely and accurate clinical documentation.

Administrative Duties

Assist with staffing coordination and scheduling as needed.
Participate in an on-call rotation.
Maintain required clinical records and reports.
Attend leadership meetings and contribute to community operations.
