Job Overview

We are seeking a dedicated and highly skilled Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong foundation in patient care, with extensive experience across various clinical settings. As an LPN, you will be responsible for providing compassionate nursing support, administering treatments, and ensuring the safety and well-being of patients. Your expertise in medical documentation, medication administration, and patient assessment will be vital in delivering high-quality care within a dynamic healthcare environment.

Duties

Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems.
Administer medications, injections, and IV infusions following established protocols while monitoring for adverse reactions.
Assist with diagnostic evaluations such as specimen collection, processing, and basic medical imaging procedures.
Provide wound care, catheterization, tube feeding, and ventilator management to patients with complex needs.
Support patients with chronic conditions such as Alzheimer's disease, dementia, and memory care requirements by implementing specialized care plans.
Collaborate with multidisciplinary teams to develop discharge planning and coordinate post-acute or long-term care services.
Maintain strict adherence to HIPAA regulations and clinical confidentiality policies during all patient interactions.
Utilize EMR/EHR systems effectively for documentation of patient care activities, medication administration records, and clinical notes.

Qualifications

Valid state licensure as a Licensed Practical Nurse (LPN) with current certification.
Demonstrated experience working in diverse healthcare settings such as long-term care facilities, hospitals—including ICU and trauma centers—and outpatient clinics.
Proficiency in Epic, Cerner, Athenahealth, or other EMR/EHR systems.
Knowledge of ICD-9/ICD-10 coding standards for accurate medical documentation and billing processes.
Critical care experience including ICU or emergency room settings is highly desirable.
Experience working with individuals with disabilities or developmental challenges; Alzheimer’s and dementia care experience preferred.
Strong understanding of anatomy, physiology, pharmacology, and medical terminology relevant to nursing practice.
Excellent patient communication skills along with the ability to perform triage assessments effectively.
Additional skills such as IV insertion, phlebotomy, suturing, aseptic technique, and infection control are advantageous.
