Full Job Description

Company Overview
Penates Inc is a dedicated homeless shelter program providing emergency housing services for single women and families referred by the Department of Social Services. Operating 24/7 year-round, we are committed to supporting individuals experiencing homelessness through compassionate care and comprehensive case management.

About the Role:
We are seeking an empathetic and highly organized Case Manager to join our dedicated team. This is a unique "temp-to-perm" opportunity designed as a working interview. You will start as a temporary employee for an initial 90-day evaluation period. If you love our team culture and excel in the role, you will be considered for a permanent, full-time position with comprehensive benefits.

Key Responsibilities:

Manage a caseload of 17 clients, conducting regular assessments and developing personalized care plans.
Coordinate with medical, social, and community resources to ensure holistic client support.
Maintain accurate, up-to-date documentation and client files in compliance with agency standards.
Advocate for clients and act as the primary point of contact for their needs.

Qualifications:

Bachelor's degree in Social Work (BSW) or related human services field.
At least 2 years of case management experience.
Strong crisis intervention skills and knowledge of local Suffolk County resources.
Exceptional communication and time-management abilities.
