Work Location

Hybrid/Remote Position with the requirement to complete visits in the community throughout Pennsylvania. Candidates must live in one of the following counties to complete home visits Philadelphia, Delaware, Montgomery, Chester and Bucks County.

Work Environment Requirements

This is a hybrid position requiring both in-office and remote work. When working remotely, employees must maintain reliable high-speed internet access to ensure consistent connectivity for meetings, systems, and communication. Failure to maintain adequate internet service may impact the ability to perform essential job functions.

We also discuss outside employment with new hires early in the hiring process to identify and prevent any potential conflicts of interest and ensure expectations are clear upon acceptance of employment.

Position Summary

The Nursing Home Transition (NHT) Coordinator plays a vital role in helping individuals safely and successfully transition from nursing facilities back to community-based living. This position works closely with participants, families, service coordinators, healthcare providers, and external agencies to ensure services and support are in place before, during, and after discharge.

The NHT Coordinator is responsible for adhering to all program requirements, maintaining timely and accurate documentation, and providing consistent participant support throughout the transition process.

EducationExperience

Associate's or Bachelor's degree in Social Work, Human Services, Nursing, Psychology, Sociology, or a related field required (Bachelor's degree strongly preferred).
Recent college graduates are encouraged to apply.
Previous experience in care coordination, case management, healthcare, or human services is preferred but not required for entry-level candidates.
Knowledge of Medicaid Waiver programs, Long-Term Services and Supports (LTSS), and Home and Community-Based Services (HCBS) is a plus.

Key Responsibilities

Serve as the primary contact for nursing home residents transitioning into community-based living through the NHT program
Coordinate transition plans with participants, families, healthcare teams, housing providers, and community agencies
Arrange housing and essential support services to ensure successful discharge and community reintegration
Monitor participant progress and maintain accurate documentation in compliance with all regulatory and organizational standards
Support outreach, quality assurance initiatives, mentorship activities, and regional travel as needed

SkillsKnowledge

Experience working with individuals with disabilities or special needs preferred
Strong skills in assessment, planning, and participant-centered service delivery
Excellent organizational, communication, and documentation skills
Ability to work effectively as part of an interdisciplinary team
Familiarity with community support systems, housing resources, healthcare, and human services
Willingness to learn and grow within the organization

Work Environment

Frequent exposure to outdoor elements and varying weather conditions
Occasional driving in poor weather conditions may be required
Noise levels are typically minimal to moderate
