Full Job Description

Position Summary

The Patient Care Coordinator provides administrative and patient support for Northwest Rheumatology, ensuring efficient front office operations, excellent patient service, and seamless communication between staff, providers, and patients. In addition to standard front desk responsibilities, this position plays a key role in proactively managing provider schedules, preparing daily appointments, coordinating patient communication, supporting transfer of care requests, and assisting with front office workflow to promote an exceptional patient experience.

Key Responsibilities

Front Desk Operations & Patient Services

Answer and route incoming calls to the appropriate department or staff member.
Greet and check in patients for in-office appointments.
Verify patient demographics, insurance, and registration information.
Collect copays, deductibles, coinsurance, and outstanding balances at check-in.
Check patients out, schedule follow-up appointments, and communicate follow-up instructions.
Assist patients with telemedicine appointments, including sending telemedicine links and troubleshooting technical issues.
Provide patient portal support, including account access and basic navigation assistance.

Schedule Management & Daily Preparation

Perform daily schedule scrubbing to prepare for upcoming appointments, verify appointment readiness, and anticipate patient needs or questions.
Monitor provider schedules and optimize appointment availability by filling cancellations and schedule gaps.
Manage cancellation lists and contact patients to fill open appointment slots.
Coordinate appointment rescheduling due to provider schedule changes or other operational needs.
Complete daily morning review of front desk voicemail messages and ensure timely follow-up.
Review the Phreesia dashboard daily to identify registration, eligibility, or payment issues requiring follow-up.
Complete front desk telephone encounters (TEs), actions, and other assigned administrative tasks promptly.

Patient Coordination & Administrative Support

Process Transfer of Care (TOC) requests according to practice policies.
Process and maintain new patient consent documentation.
Contact patients to verify mailing addresses for refund checks and returned mail.
Receives incoming mail and processes mailed patient payments.
Assist with incoming faxes and document importing as needed for department coverage.
Maintain accurate documentation within the electronic health record (EHR).

Qualifications

Previous experience in a medical office or healthcare setting preferred.
Knowledge of electronic health record (EHR) systems and medical office workflows.
Excellent customer service, communication, and interpersonal skills.
Strong organizational skills with the ability to prioritize multiple tasks in a fast-paced environment.
High attention to detail and accuracy.
Ability to maintain patient confidentiality and comply with HIPAA regulations.
Ability to problem-solve independently while working collaboratively with the healthcare team.
Team-oriented with a willingness to cross-train and provide coverage across front office functions.

Schedule & Work Environment

Full-time, on-site position.
Work closely with providers, medical assistants, billing staff, and administrative personnel in a fast-paced, patient-centered medical practice.

Pay: $21.00 - $23.00 per hour
