Full Job Description

Job Summary – Housing Navigator

Housing Navigators assist participants throughout the process of locating and obtaining permanent supportive housing. As ECS continues to collaborate with the Department of Homelessness and Supportive Housing and other external stakeholders in launching this project, this job description may change both prior to hire and afterward, as we refine and implement the most effective approaches and services.

Primary Duties and Responsibilities:

Work with participants as assigned by the Manager of Housing Navigation Services.
Conduct outreach to pre-identified individuals experiencing homelessness, providing professional and culturally responsive support, advocacy, information, and referrals to assess eligibility and guide them through the housing application process.
Maintain relationships with participants eligible for housing placement throughout the placement process.
Assist participants in completing housing applications, including gathering required documents, attending interviews, and supporting them in grievance processes if applications are denied.
Support participants with moving into new housing once applications are approved.
Assist tenants with completing housing provider certification and re-certification processes as required.
Develop and maintain relationships with property managers, leasing agents, and support services staff at housing referral sites.
Keep accurate, up-to-date records and charts in the HSH One System, ensuring participant privacy and confidentiality.
Provide professional, accessible, and culturally responsive support, advocacy, information, and referrals to program participants.
Provide referral services as needed.
Work within and promote the integrated services team model in an environment dedicated to harm reduction, recovery, and wellness.
Maintain a strong understanding of housing-related resources and services available in the community, especially for participants with acute medical and behavioral health needs (e.g., rental assistance programs, IHSS services, food programs, and health/behavioral health services).
Participate in regular Coordinated Entry team meetings.
Provide crisis management support to participants experiencing acute behavioral health symptoms as needed.
Collaborate with outside agencies and teams to support participants in securing housing and essential resources, while helping them develop self-advocacy skills.
Participate in ongoing training provided by ECS, HSH, and other partners as appropriate.
Maintain confidentiality of participant health information in compliance with HIPAA and all confidentiality laws. Consult with a supervisor as needed regarding exceptions.
Follow policies, procedures, and protocols established by HSH and Episcopal Community Services.
Maintain cooperative and collaborative working relationships with ECS employees, volunteers, participants, and host shelters.
Perform other duties as required.

Requirements:

Master’s or Bachelor’s degree in psychology, counseling, social work, or a related field preferred; OR at least 3 years of relevant experience in a closely related role.
Demonstrated knowledge and experience working with individuals in crisis or unstable housing situations.
Strong knowledge of issues faced by, and sensitivity to, adults with substance use disorders, domestic violence, HIV/AIDS, mental health issues, histories of trauma, and co-occurring diagnoses.
Respect for, experience with, and willingness to work with a diverse participant population.
In-depth knowledge of resources available to homeless and low-income residents of San Francisco.
Strong verbal and written communication skills; able to communicate complex information clearly.
Ability to deliver culturally competent services and work effectively across diverse populations.
Professional, discreet demeanor with strong interpersonal skills; able to handle confidential information with tact, respect, discretion, and judgment.
Ability to organize information, materials, and workflows systematically to maximize efficiency and avoid duplication of effort.
Experience working in integrated service teams using participant-centered, trauma-informed, harm-reduction approaches.
Collaborative work style; able to work independently and as part of a team to achieve successful outcomes.
Strong computer skills; proficiency in Microsoft Office required.
Knowledge of current permanent supportive housing eligibility requirements, including HUD homeless regulations, is a plus.
Familiarity with the ONE System preferred.

Flexibility and adaptability to evolving Coordinated Entry activities, lessons learned, and emerging community needs.

Other Requirements:

Must pass fingerprint screening and annual TB screening.
Must meet the physical requirements of the position, which may include long periods at a desk and/or computer workstation, daily travel to various sites, and lifting up to 25 lbs.
