Full Job Description

PrimeSTAR Home Health is seeking a Full-Time RN Clinical Care Manager (office) and a Full-Time Field RN (on the road) to join our team of one of the fastest-growing Home Healths in North Louisiana. This position serves patients in Lincoln Parish, Jackson Parish, Union Parish and surrounding areas. This position is located in our Ruston office.

Duties & Responsibilities For Clinical Care Manager: We are currently seeking an energetic Registered Nurse/Clinical Manager to assist the agency with the coordination of home health services to patients in their place of residence in accordance with physician orders. The RN Clinical Care Manager is under the supervision of the Director of Nurses. The RN Clinical Care Manager is available all times during operating hours and participates in all activities relevant to professional services furnished, including the management of patient care. Must be able to manage agency activities with DON not present. The RN Clinical Care Manager will participate in developing standards which ensure safe and therapeutically effective service to patients and families. Ability to provide oversight in a positive manner to team members in order to meet the needs of the agency.

Duties & Responsibilities for Full-Time Field RN:

patient teachings
medication administration
wound care / dressing changes / wound vacs
post-surgical care
skilled nursing care / direct patient care
patient follow-up
OASIS Start of Care (SOC), regular SOC, admits & discharges
Willingness & ability to drive throughout our territory
Additional duties as assigned

Required experience:

We are seeking Registered Nurses (RN) whose license is active and in good standing with the Louisiana Board of Nursing
Excellent clinical nursing skills and a background in ICU, ER, med/surg, or related skilled nursing care
Experience with Home Health nursing and Medicare / OASIS a plus
Willingness & ability to drive throughout our territory
Other duties as assigned

Pay: $75,000.00 - $80,000.00 per year
