Employment Details: Full Time, Non- Exempt, Monday - Friday during business hours

Compensation: $27-29.50/hour

About the role: We are hiring a Case Manager for our Domestic Violence Rapid Rehousing Program! This is an excellent opportunity to gain hands-on case management experience while supporting individuals and families experiencing homelessness. With a manageable caseload of approximately 15 households, you will provide compassionate, wraparound services to participants who have fled or are attempting to flee domestic violence situations, helping them achieve housing stability and long-term success.

What you’ll do:

Conduct participant screenings, verify eligibility, and complete program assessments to determine service needs and housing barriers.
Develop individualized housing stability plans with participants, setting achievable goals and connecting them to appropriate resources and support services.
Provide strengths-based case management, financial coaching, and ongoing support to help participants achieve long-term stability and self-sufficiency.
Coordinate referrals to SHELTER, Inc. programs and community partners, advocating for participants and ensuring access to critical services.
Maintain a caseload of approximately 15 households, facilitating regular check-ins, care coordination, and progress monitoring.
Reassess participant needs and adjust service plans as necessary to support successful housing outcomes.
Manage program enrollments, transitions, and exits in accordance with agency policies and funding requirements.
Accurately document services, case notes, and participant outcomes in HMIS and other required systems, ensuring timely and compliant recordkeeping.

What you’ll bring:

1+ years' experience working with low income and/or homeless populations in wraparound services.
Bachelors or Associate's degree in social services or a related field preferred. Equivalent experience will be considered in lieu of or in combination with formal education will be consider.
Proficient computer skills, including Microsoft Office Suite (Outlook, Word and Excel)
Experience working in diverse settings with people across all socio-economic spectrums and a wide variety of personalities and roles - staff, residents, local agencies, contractors, lenders, etc.

Position Requirements:

This position requires frequent driving. A valid California driver license, reliable personal vehicle, current personal auto insurance as required by law.
Must successfully pass a criminal background check
Must provide TB test clearance
