Licensed Therapist

POSITION TITLE: Licensed Therapist

SUPERVISED BY: Executive Director

FLSA STATUS: Full time, benefited position

REVIEW DATE: March 2025

Community Health Resource Center (CHRC) is a nonprofit 501 (c) (3) corporation. Since 1986 CHRC’s mission has been to provide and promote innovative health care and health maintenance programs to the greater Bay Area as well as the patients served by the Sutter Health/California Pacific Medical Center. CHRC provides counseling, education and coordination of multiple patient services to supplement the care and support provided by physicians.

Position Description: Under the direction of the Executive Director to, the licensed clinical social worker, licensed marriage and family therapist, licensed psychologist (PhD or PsyD). We are building a Collaborative Care Team dedicated to integrating behavioral health into primary care settings. Our goal is to provide short-term, evidence-based, and patient-centered care by working closely with physicians, PAs, nurses, and other healthcare professionals in a primary care setting. Role Overview:

As a Licensed Therapist (LMFT, LCSW, PhD, PsyD) on our Collaborative Care Team, you will provide diagnostic assessments, brief structured interventions, consultation, and care coordination in an integrated primary care setting. This role requires strong clinical skills, adaptability, a learning mindset, and a commitment to team-based care to improve patient outcomes.

Key Responsibilities

Conduct diagnostic assessments to support treatment planning.
Provide brief, structured interventions (e.g., grounding techniques, diaphragmatic breathing, CBT-based coping strategies) to support emotional regulation and well-being.
Utilize measurement-based care (e.g., PHQ-9, GAD-7) to track progress and inform treatment.
Participate in interdisciplinary case consultations with medical and behavioral health teams.
Provide psychoeducation to patients, families, and healthcare providers to improve mental health awareness and engagement.
Maintain accurate documentation in compliance with HIPAA and professional ethical standards.
Contribute to program development by refining workflows, protocols, and best practices in collaborative care.

Qualifications

Active, unrestricted California license as an LMFT, LCSW, PhD, or PsyD.
2+ years of clinical experience (preferred).
Spanish fluency (preferred).
Experience in integrated care, primary care, or medical settings (preferred).
Familiarity with evidence-based modalities (CBT, ACT, DBT).
Strong collaboration and communication skills to work effectively within a multidisciplinary team.
Ability to thrive in a fast-paced environment and adapt to evolving patient and system needs.
Commitment to culturally competent, trauma-informed care, including work with underserved and LGBTQ+ communities.

Computer Skills:
