Community Health Worker II (Bilingual Spanish) | Full-Time

Make a Difference in Your Community

Seeking a compassionate, bilingual Community Health Worker II to support underserved individuals and families throughout Alameda and Santa Clara Counties. This is an excellent opportunity for a community-focused professional who is passionate about helping patients overcome barriers to care and access the resources they need to thrive.

As a trusted liaison between patients, healthcare providers, and community resources, you will play a critical role in improving health outcomes and quality of life for diverse populations.

Position Overview

Position: Community Health Worker II
Modality: Non-Clinical Case Manager
Schedule: Full-Time, 40 Hours Per Week
Shift: Day Shift
Days: Variable Schedule, Including Weekends
Assignment Length: Long-Term Contract
Start Date: ASAP
Interview Required: Yes

What You'll Do

Serve as a trusted resource and advocate for patients and families
Conduct outreach and community engagement activities
Assist patients in navigating healthcare systems and available services
Coordinate care between primary care providers, specialty providers, and community organizations
Support case management activities and follow-up services
Connect patients with critical resources including:
- Food assistance
- Housing resources
- Financial support programs
- Transportation services
- Community-based services
Educate patients on available healthcare and social service programs
Help reduce barriers to care and improve patient access to services
Participate in community events and outreach initiatives
Maintain professionalism and compassion while serving diverse populations

Required Qualifications

Passion for serving underserved and low-income communities
Desire to work in a community-based healthcare setting
Ability to build trust and establish positive relationships with patients
Strong communication and interpersonal skills
Ability to work independently and collaboratively within a team environment
Experience coordinating services and community resources preferred

Preferred Qualifications

Bilingual Spanish strongly preferred
Additional language skills highly valued
Previous Community Health Worker, Case Management, Human Services, Social Services, Outreach, Care Coordination, or Patient Navigation experience preferred
Epic experience preferred
Experience working with diverse cultural and socioeconomic populations preferred

Orientation & Training

First week includes required onsite orientation and training
Orientation begins at the Fremont Clinic
Attendance at orientation is mandatory

Compensation

Local Pay Rate: $25.00 per hour

The Non-Clinical Case Manager coordinates referrals, service planning, and client documentation for designated caseload(s).

Minimum Requirements:

Professional License in Behavioral Sciences, Human Services, or Social Services preferred
Bachelor’s Degree in Behavioral Sciences, Human Services, or Social Services fields preferred
Experience in Case Management, Behavioral Sciences, Human Services or Social Services preferred
Complies with all relevant professional standards of practice
Current CPR if applicable
TB questionnaire, PPD or chest x-ray if applicable
Current Health certificate (per contract or state regulation)
Must meet all federal, state and local requirements
Must be at least 18 years of age
