About the Organization

Lutheran Social Services of Northern California (LSS) supports dignity and stability for our community’s most vulnerable by providing supportive housing services that lead to self-sufficiency. LSS’ service area is Northern California and we maintain offices in Concord, Sacramento, San Francisco, Redding and Stockton.

The position is part of the team for the MiCasa program which is a two year transitional housing program for young adults who have emancipated from foster care and become homeless. Supportive services are provided to promote self-sufficiency and wellness. The program utilizes a strength-based and trauma informed care approach, helping participants rely on and increase their internal resources and linking them to supportive relationships and resources outside themselves.

Required Skills and Experience

BA or BS degree from an accredited college or university, preferably with a major in a behavioral science, and 4 years experience in the human services field.
Maintain a strength-based perspective.
Able to work with a variety of community organizations and health providers.
Treat others with respect and courtesy, striving for open and honest working relationships.
Ability to work with adults or youth with low income from diverse social and ethnic backgrounds who have a history of homelessness.
Demonstrated knowledge of Trauma Informed treatment framework.
Professional experience working with clients who have AOD dependencies, mental health diagnoses, domestic violence histories, issues with abandonment, and abuse.
Ability to access community-based services and to collaborate with other service providers.
Has a clean driving record, licensed and registered car, and proof of insurance.

Essential Duties and Responsibilities:

Outreach and engagement of potential clients
Comprehensive assessment and development of the care management plan
Enhanced care coordination
Health promotion
Comprehensive transitional care
Member and family support
Coordination and referral to community social support services
Care management documentation

Job Type: Full-time M-F, 10am to 6pm
