Full Job Description

Gateway Housing First (GHF) seeks a Supportive Housing Case Manager with a substance use recovery specialty to advance our Mission through innovative and ever-evolving permanent supported housing program development and implementation.

We seek passionate advocates who embrace collaboration and want to meaningfully support policy change and systems improvements on behalf of those who are not equitably and effectively served by our existing systems of care.

Gateway Housing First’s mission is to provide safe and secure supported housing to individuals and families who struggle most to secure and maintain housing, who are challenged by a wide range of disabilities and complex disorders, and who are also impacted by chronic homelessness, generational poverty, and too often, systemic racial inequities. We execute our mission in accordance with our core values that: (1) all people have the right to affordable housing, to choose where they live and obtain a lease in their own name with all the rights and responsibilities of tenancy; (2) housing is an essential component of an effective healthcare delivery system; and (3) people are inherently housing ready and will achieve improved housing stability when able to access right-fit housing and supports that are relevant and responsive to their needs, as they articulate them.

Compensation will be commensurate with experience and skills.

Position Summary

The PSH Case Manager (Substance Use Recovery Specialist) provides intensive, community-based case management services to individuals in GHF housing who have experienced homelessness, have severe substance use disorders and co-occurring mental health conditions. Utilizing Housing First, Motivational Interviewing, and Trauma-Informed Care principles, this position supports tenants in maintaining permanent housing, improving health and wellness, reducing barriers to stability, and pursuing self-defined recovery goals. Success in this role is measured by housing retention, tenant engagement, improved health and wellness outcomes, and increased connection to treatment and community supports.

Qualifications

Education

Bachelor's degree in Social Work, Psychology, Counseling, Human Services, or a related field required.

Experience

Minimum of at least one year of experience working with adults experiencing homelessness, serious mental illness, substance use disorders, or other complex barriers to housing stability.

Preferred Credentials

Certified Alcohol and Drug Counselor (CADC) or equivalent state-recognized addiction credential preferred.

Knowledge & Skills

Demonstrated knowledge of Housing First, Trauma-Informed Care, Motivational Interviewing and substance recovery evidence-based practices.
Strong assessment, engagement, crisis intervention, and case management skills.
Ability to work effectively with individuals experiencing chronic homelessness, behavioral health conditions, and substance use disorders.
Strong written, verbal, organizational, and documentation skills.
Ability to work independently while contributing effectively as part of a multidisciplinary team.

Other Requirements

· Valid driver's license, reliable transportation, and proof of automobile insurance.

· Ability to conduct home visits and community-based services throughout the service area.

Key Responsibilities

· Caseload Management & Housing Stability

Maintain an active caseload of approximately 25 tenants.
Conduct regular home visits and community-based outreach.
Assist tenants in obtaining documentation required for housing applications, certifications, and subsidy renewals.
Support housing move-ins, conduct intake and orientation, and successful transition into permanent housing.
Support tenants in maintaining lease compliance, keeping utilities current and positive landlord relationships.
Address tenancy concerns in collaboration with property management, service providers, and other community partners.

Individualized Service Planning

Develop, implement, and regularly update individualized Housing and Service Plans with tenant participation.
Support tenants in achieving goals related to housing stability, income, health, recovery, and community integration.
Assist tenants in obtaining and maintaining employment, benefits, and other income sources.
Connect tenants to healthcare, behavioral health, and wellness services.
Support development of independent living skills, household management skills, and natural support networks.

Substance Use & Recovery Support

Provide evidence-informed substance use interventions.
Deliver education on overdose prevention, safer use practices, relapse prevention, and recovery resources.
Conduct screenings and assessments as appropriate.
Coordinate referrals to detoxification, outpatient treatment, inpatient treatment, medication-assisted treatment (MAT), peer recovery supports, and other recovery services.
Support and encourage tenants in pursuing self-defined recovery goals while respecting individual choice and autonomy.

Crisis Intervention & Advocacy

Respond appropriately to behavioral health, substance use, and housing-related crises.
Utilize de-escalation techniques and connect tenants with emergency and crisis services when needed.
Accompany tenants to medical, psychiatric, social service, and court-related appointments as appropriate.
Advocate for tenants' access to services and resources within healthcare, housing, and social service systems.

Documentation & Compliance

Maintain complete, accurate, and timely tenant records in accordance with agency, funder, and regulatory requirements.
Document all tenant contacts and services provided.
Enter and maintain data in HMIS and other approved databases.
Track and report tenant outcomes and program performance measures.
Participate in case reviews, team meetings, supervision, and required trainings.
