Full Job Description

Job Overview
We are seeking a dedicated and compassionate Occupational Therapist to join our team. The ideal candidate will possess a strong understanding of patient care and rehabilitation techniques, with experience working across various settings including pediatrics, geriatrics, and acute care. This role involves collaborating with patients to develop personalized care plans that enhance their ability to perform daily activities and improve their overall quality of life.

Responsibilities

Assess patients' needs and develop individualized treatment plans focusing on improving daily living skills (ADLs).
Implement therapeutic interventions aimed at enhancing functional abilities in patients with disabilities or chronic conditions.
Conduct discharge planning to ensure a smooth transition for patients returning home or moving to other care settings.
Collaborate with multidisciplinary teams, including speech therapists and case managers, to provide comprehensive patient care.
Document patient progress and modify treatment plans as necessary based on ongoing assessments.
Provide education and support to patients and their families regarding treatment goals and strategies for continued improvement.
Work with diverse populations, including individuals with autism, in both inpatient and outpatient settings.
Utilize knowledge of physiology, anatomy, and medical terminology to inform treatment decisions.

Requirements

Degree in Occupational Therapy from an accredited program.
Valid NYS licensure as an Occupational Therapist.
Experience in various settings such as skilled nursing facilities, home health, outpatient clinics, or schools is preferred.
Knowledge of case management principles and the ability to develop effective care plans.
Familiarity with early intervention strategies for pediatric populations.
Experience working with geriatric patients and understanding their unique needs is a plus.
Strong communication skills for effective collaboration with healthcare teams and patient education.
