Full Job Description

JOB DESCRIPTION SUMMARY-

As a hospice scheduler, by scheduling visits, the work you do every day makes a difference in the lives of our patients by assisting the field staff to provide exceptional care. You’ll prepare & adjust day-to-day schedules for the field staff and courteously respond to all patient inquiries and problems, which helps add to the positive patient experience. Responsible for ordering, delivery & tracking of patient medical supplies.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES-

Coordinate and maintain the patient visit schedule for hospice clinical staff.
Adjust and update schedules daily, weekly, and monthly based on patient needs and staff availability.
Communicate schedule changes with clinical staff and department managers to ensure continuity of care.
Schedule aide care plan which is to be completed by a registered nurse
Works in close coordination in hospice clinicals regarding on going visits
Assist in answering phone calls
Order, distribute & track patient medical supplies
Use Home Health Notify to notify and communicate with staff
Backup front desk & intake, when needed.
Assist business office administrators in faxing histories, physicals, physician orders, and notification of hospice
Email/fax IDG updates to facilities as they are requested
Assign and Track aides CEU’s
