Full Job Description

The MDS Coordinator (RN) is responsible for managing and coordinating the Resident Assessment Instrument (RAI) process to ensure timely and accurate completion in compliance with Title 22, OBRA regulations, CMS guidelines, and California Department of Public Health (CDPH) standards. This role is pivotal in driving quality outcomes through comprehensive resident assessments, care planning, and interdisciplinary collaboration. The MDS Coordinator plays a key role in influencing the facility’s CMS 5-Star Quality Measures by ensuring data integrity and regulatory compliance.

II.

A. Demonstrates advanced physical and psychosocial assessment skills for initial and ongoing appraisal of resident status.

B. Leads the development and implementation of individualized, comprehensive care plans based on resident assessments and clinical data.

C. Maintains and coordinates the RAI process, ensuring timely and accurate completion of the Minimum Data Set (MDS) in accordance with the RAI Manual and regulatory requirements.

D. Identifies and responds to Significant Changes in Status, ensuring appropriate updates to assessments and care plans.

A. Ensures documentation and coding accuracy aligned with CMS guidelines, CDPH regulations, and OBRA standards.

B. Monitors and analyzes MDS data to support facility performance on CMS 5-Star Quality Measures, including Quality, Staffing, and Health Inspection domains.

C. Participates in Quality Assurance and Performance Improvement (QAPI) initiatives related to resident assessment and care planning.

A. Facilitates and actively participates in Interdisciplinary Team (IDT) meetings and resident care conferences.

B. Serves as a resource to IDT members on RAI process, MDS coding, and care planning best practices.

C. Implements recommendations from IDT and ensures follow-through on care interventions.

A. Submits written documentation which consistently meets guidelines within the time frames established by the facility.

B. Follows documentation procedures as required by RAI manual, Title XXII and OBRA guidelines. Uses Accurate terminology.

C. Inputs and validates MDS data in electronic health record systems.

D. Utilizes clinical software tools to track assessments, care plans, and quality indicators.

A. Stays current with updates to the RAI Manual, CMS regulations, and CDPH guidelines through ongoing education and training.

B. Provides mentorship and training to nursing staff on MDS processes and care planning.

C. Demonstrates leadership in promoting a culture of compliance, quality, and resident-centered care.

D. Attends workshops, seminars or classes to further develop professional knowledge/skills by seeking or accepting new or challenging assignments when requested.

E. Establishes professional goals in coordination with Director of Nursing and shows progress towards achievement.

F. Participates in internal committees/projects when requested and participates in staff meetings on a regular basis.

IV.

Graduation from an accredited nursing program. Valid California registered nursing license.

Two years recent skilled nursing experience and one year working with the RAI preferred. RAC Certification preferred.

A. Demonstrates expert-level understanding of the Resident Assessment Instrument (RAI) process, including accurate completion of the Minimum Data Set (MDS) and Care Area Assessments (CAAs), in alignment with the RAI User’s Manual.

B. Possesses strong knowledge of comprehensive nursing care, including clinical disease processes, physical and psychosocial assessment techniques, and evidence-based care planning.

C. Maintains up-to-date knowledge of Centers for Medicare & Medicaid Services (CMS) guidelines, California Department of Public Health (CDPH) regulations, and Title 22 requirements governing skilled nursing facilities.

D. Understands and applies principles of CMS 5-Star Quality Rating System, including Quality Measures, Staffing, and Health Inspection domains, to support facility performance and resident outcomes.

E. Demonstrates proficiency in identifying and responding to significant changes in resident status, initiating appropriate reassessments and updates to care plans.

F. Skilled in interpreting diagnostic data and integrating findings into individualized, goal-oriented care plans.

G. Capable of leading and facilitating interdisciplinary team (IDT) meetings and resident care conferences, ensuring collaborative and person-centered care planning.

H. Proficient in the use of electronic health record (EHR) systems for MDS data entry, validation, and reporting.

I. Able to analyze MDS data to identify trends, support quality improvement initiatives, and ensure compliance with regulatory benchmarks.

J. Maintains a high level of professionalism, clinical judgment, and ethical standards in all aspects of resident care and documentation.

Sedentary Work

Exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.

Heavy Work

Exerting over 50 pounds of force occasionally, and/or up to 50 pounds or force frequently, and/or up to 20 pounds of force constantly to move objects. If related to patient care - Must be able to safely assist residents/patients including but not limited to lifting, turning, supporting weight and movement.
