Full Job Description

GENERAL SUMMARY: Ensures the delivery of quality nursing care. Evaluates individual status, directs/supervises ancillary staff and adjusted nursing care priorities. Oversees and/or provides care to individuals utilizing prescribed long-term care treatment modalities. Acts as role model and preceptor to new or less experienced staff members, is knowledgeable regarding standard facility policies, operating procedures and regulatory guidelines. Make individual health care nursing plans that includes admission, oversight of medication management, therapies & treatments, evaluation of test results & an evaluation of the plan’s effectiveness. Act as liaison between individual and their families, guardians, advocates, health care personnel, the agency and others as required. Assist with medical, dental vision and/or specialty appointments within the standards of Federal, State or local regulations, whichever is more stringent (usually 30 days) to ensure continued efficient, smooth and prompt health care services.

Qualifications: Must be able to accurately & timely complete job duties with little supervision. Must be detail oriented. Must be able to work with individuals with all levels of disabilities. Must meet all local health regulations, pass background checks, drug testing and satisfactory employment verifications. Knowledge of current & established nursing concepts, principles and practices. Knowledge & understanding of Infection Control, Universal Precautions, Incident Reporting, Care Planning and skills to develop & implement nursing standards of practice. Ability to organize and manage multiple priorities. Possess basic knowledge of certification requirements of CNA’s and CMT’s as set forth by the MBON and DDA MTTP process.

Essential Responsibilities:

•Maintain RN license, CPR certification, and auto insurance.

•Obtain RN/DN certification, online HRST Rater training, online HRST Advanced Rater training, and Online HRST Reviewer training.

•Attend three (3) mandatory Developmental Disabilities Administration (DDA) nurse quarterly meetings per year.

•Serve as the Delegating Nurse/Case Manager for each client assigned.

•When the client is first assigned, visit the client in the environment in which delegation occurs, conduct and complete an initial clinical assessment and develop the nursing plan of care.

•Complete Choking Risk Screening Tool and Feeding Protocol as necessary.

•Every 45 days, visit clients in the environment in which delegation occurs to assess their physical condition, review medications, documentation, prescriber orders, and review medical appointment/screening exams/immunizations/lab work as needed, and provide necessary care.

•Review and update physician’s orders every 90 days.

•Participate in PCP meetings when needed.

•Visit clients who are not chronic, stable, routine, uncomplicated, and predictable minimally every 2 weeks in the environment in which delegation occurs.

•May have to attend unscheduled emergency visits to individuals either in home, Urgent Care or Emergency Room.

•Conduct Interim visits for clients in their residence and/or day/vocational/supported employment setting as needed for occurrences such as post-ER visits, after hospital discharge, dressing changes, falls, injuries, etc. Visit must be completed within 48 hours of incident.

•Delegate administration of medication and other delegable tasks to certified Medication Technicians.

•Train and ensure staff’s competency in nursing care plans, delegated nursing tasks, feeding protocols (if applicable), and any other necessary tasks, protocols, or forms.

•Review electronic Medication Administration Records (MAR’s).

•Ensures that prescribed medications are appropriately secured and dispensed correctly.

•Monitor and report medication errors made by residence staff to supervisor.

•Conduct medication administration practical exams for Medication Technicians.

•Complete, review and update online HRSTs as needed.

•Operates facility equipment safely and effectively and is knowledgeable regarding standard procedures for maintenance of nursing equipment.

•Document all nursing interventions/nursing care in writing and within appropriate time frame.

•Assess staff performance levels and communications findings to program directors.

•Participates with other allied health disciplines in individual care conference aimed at meeting the needs of the total individual and family.

•Develop and monitor nursing care plans for individuals and provide to the certified nurse’s aide, and certified medication technicians at the homes for clinical direction and assistance as appropriate.

•Interact with attending physicians and make recommendations for changes in care based on results of nursing assessments. Initiates physician’s orders and appropriate nursing interventions as needed.

•Actively participates in staff education programs. Identifies program topics.

•Conducts informational educational classes as the need arises, in order to orient staff to new equipment, procedures and treatment.

•Identifies, resolves, reports and records unusual incidents or occurrences in accordance with standard operating procedures.

•In the event that the United States Government and/or the Local State Government determines and announces the decision to close, delay or cancel activities in the State of Maryland related to emergency conditions, this position is required to complete all regular duties or as instructed by management.

•Attend scheduled Nurse and staff meetings.

•Gives/receives report upon reporting in and ending of shift to supervisor.

Qualifications:

•BS Degree in Nursing.

•Current/valid Maryland RN license

•CPR/First Aid

•Experience in the developmental disabilities’ environment is highly desired.

•Must have current/valid state –issued driver’s license and own reliable transportation.

•Must be able to walk up and down stairs.
