Full Job Description

Position Summary:
The Patient Navigator plays a key role in supporting patient access and care coordination across a multispecialty ambulatory network. This position serves as a central point of contact for patients, providers, and administrative teams, ensuring seamless scheduling, accurate data management, and effective communication between hospital departments and multiple outpatient offices. The Patient Navigator helps optimize patient flow, supports operational efficiency, and enhances the overall patient experience.

Key Responsibilities:

Serve as a primary contact for patient inquiries, providing timely, professional, and empathetic assistance via phone and other communication channels
Coordinate and schedule patient appointments across multiple specialties, locations, and hospital departments, ensuring alignment with provider availability and patient needs
Facilitate communication and scheduling coordination between the hospital and affiliated ambulatory offices
Manage and monitor scheduling workqueues and tracking spreadsheets to ensure timely follow-up, accurate appointment status, and resolution of outstanding items
Conduct proactive patient outreach for appointment scheduling, confirmations, reminders, and care coordination needs
Maintain accurate and up-to-date documentation in the electronic medical record (EMR) and other tracking systems
Generate, review, and distribute reports related to patient scheduling, access metrics, and operational performance across multiple sites
Ensure the accuracy and integrity of patient data, including maintaining detailed records of scheduled appointments and outreach efforts
Collaborate with clinical and administrative teams to identify scheduling barriers and implement process improvements
Support data dissemination and communication across departments to promote consistency and alignment

Qualifications:

High school diploma or equivalent required; Associate’s or Bachelor’s degree preferred
2+ years of experience in healthcare administration, patient access, or care coordination, preferably in a multispecialty or ambulatory setting
Strong understanding of medical scheduling processes and healthcare operations
Experience working with EMR systems and scheduling/workqueue tools
Proficiency in Microsoft Office, particularly Excel, for tracking and reporting

Skills and Competencies:

Ability to manage multiple priorities and work in a fast-paced environment
Strong communication and interpersonal skills with a patient-centered approach
Analytical mindset with the ability to interpret data and maintain accurate records

Work Environment:
This role is based in an ambulatory, multispecialty healthcare setting and may require coordination across multiple locations and departments.
