Job Overview

We are seeking a dedicated and compassionate Participant Care Coordinator to join our dynamic healthcare team. In this vital role, you will serve as a primary point of contact for participants, ensuring their needs are met with professionalism, empathy, and efficiency. Your expertise will help coordinate comprehensive care plans, facilitate communication between participants and healthcare providers, and support the delivery of high-quality patient-centered services. Job Summary:

The Participant Care Coordinator will serve as an ongoing point of contact between Always With Love Home Care Agency, participants, responsible parties, caregivers, managed care organizations, and internal departments.

The position is responsible for maintaining participant relationships, monitoring service delivery, identifying and following up on concerns, conducting participant visits, monitoring Plans of Care and authorized services, and ensuring participant matters are appropriately documented, resolved, or escalated

Supervisory Responsibilities:

None

Duties/Responsibilities:

-Managing a large assigned participant caseload.

-Conducting routine participant and responsible-party check-ins.

-Maintaining regular communication with caregivers regarding participant needs.

-Monitoring participant satisfaction and overall service delivery.

-Reviewing Plans of Care and authorized services with patients.

-Monitoring authorization deadlines, renewals and service changes.

-Identifying changes in participant needs.

-Communicating identified changes to appropriate internal departments, supervisors, MCOs or other appropriate entities.

- Assisting with participant concerns.

Following up on complaints and escalating concerns when appropriate.
Maintaining detailed documentation of participant contacts, concerns and resolutions.
Monitoring EVV issues when they affect participant service delivery.
Coordinating with Intake, EVV, HR, Payroll and management.
Reporting incidents and significant participant changes in accordance with agency procedures.
Maintaining accurate and confidential participant records.
Supporting quality-assurance and compliance activities.
Ensuring outstanding participant needs receive timely follow-up.
The candidate must be willing and able to conduct scheduled and as-needed in-home participant visits.
During visits, the Participant Care Coordinator may be responsible for:

Assessing participant satisfaction and overall service delivery.
Confirming services are being provided in accordance with the Plan of Care.
Observing the home-care environment for service or safety concerns.
Discussing services, schedules, concerns and changing needs with participants and caregivers.
Documenting observations, participant feedback, concerns and follow-up actions.
Reporting changes in participant condition or care needs.
Following up on complaints or caregiver performance concerns.
Conducting additional visits when directed by management or when circumstances require additional follow-up.Required Skills/Abilities:

Must be bilingual – English and Spanish
Excellent verbal and written communication skills, including clear verbal and written communication with clients, families, and internal teams.
Must have a valid driver’s license, candidate will be required to drive regularly in their plum duties.
Excellent interpersonal skills with the ability to manage sensitive and confidential situations with tact, professionalism, and diplomacy.
Excellent organizational skills and attention to detail.
Proficient with or the ability to quickly learn payroll management, human resource information system (HRIS), and similar computer applications.
Strong interpersonal skills to build trust, demonstrate empathy, and support clients and families during the onboarding process.
Ability to identify when an issue requires escalation.
Ability to work independently in the field.
Comfort interacting directly with participants and caregivers in their homes.
Ability to communicate across multiple departments.
Experience in customer or client onboarding, with the ability to manage multiple onboarding cases and ensure timely completion of required steps.
Detail-oriented, with the ability to manage priorities in a fast-paced environment.
Comfort using computer systems and software for data entry, documentation, and communication; experience with client management systems (CRMs) is a plus.
Ability to work on-site in Bensalem, PA, and collaborate effectively with cross-functional teams.

Education and Experience:

High School Diploma required, Associates or Bachelor’s degree preferred.
Software such as HHA Exchange & GIMBAL are preferred.
Valid Driver’s License Required- Initial and annual driving-record/MVR review will be completed before permitting the employee to operate an agency vehicle.
Proficient in MS Office Suite (Word, Excel, Email).
Previous experience in administrative healthcare, home care, social services, or a related client-facing field is preferred.

· Case-management support.

· Customer service experience, healthcare customer service preferred

· Care coordination.

· Managed-care or authorization experience.

· Experience communicating with caregivers and families.

Prior related office experience preferred.

Physical Requirements:

Prolonged periods of sitting at a desk and working on a computer.
Must be able to drive when needed throughout the employee shift
Must be able to lift up to 15 pounds at times.
