Full Job Description

Haven Adoptions & Family Services (Haven) is a licensed adoption and foster care agency in the Commonwealth of Pennsylvania. For over 12 years, Haven has worked with hundreds of adoptive families, foster care families, and children. Haven was founded to create a new standard in the field of adoption and foster care. Haven’s private adoption program works with pre-adoptive families to educate, prepare, and approve before becoming an active waiting family ready to match with an expectant parent. Our goal is to provide hope and support to everyone we work with, and Haven puts the needs of our clients first. Haven has a solid reputation in both the adoption field and foster care field. We are known for the highest level of personal service, support for clients, and kindness and compassion to all of our families and children. The primary role of an adoption case manager is to support the pre-adoptive and adoptive families who are in our private adoption program. This entails direct case management, collaborating with the Director of Family Services, supporting each family thorough their adoption journey, in person support, and regular email and phone contact.

Job Details:

· This position will report to the Director of Family Services.

· Represent and uphold Haven’s missions to other agencies, organizations and the general public.

· Provide direct support to help the pre-adoptive and adoptive families in our private adoption program.

· Keep all necessary paperwork within personnel file up to date.

·Job Type: Full-time

Specific Duties to include:

Onboard new private adoption families - create Dropbox folder, subfolders, contact cards, and keep all necessary files for clients up to date.
Create a board for each new family, log case notes, updates, ongoing communication.
Participate in introductory Zoom meetings with Director of Family Services and each family.
Check in weekly when families have first joined and then regular checkins along the way.
Monitor training for families throughout the adoption process (combination of prerecorded in house trainings and Creating a Family).
Provide emotional support to all adoptive families via Zoom and phone calls.
Prepare and educate adoptive family in each phase of the adoption process (waiting, matching, preparing for hospital, post adoption contact, first days home, and post placement period).
Help prepare and send matching opportunities to waiting families.
Attend initial in person “meet and greet” meetings between expectant parents and pre-adoptive family with Biological Parent Case Manager at time of matching.
In person support when baby is born as needed - work directly with Biological Parent Case Manager.
In person presence at hospital discharge when baby and adoptive family leave, along with review and execution of necessary legal documents with adoptive family.
Participate in weekly adoption supervision/case review.
Participate in weekly adoption team meetings.
Complete home studies and post placements for full service adoptive families.
Collaborate with Home Study and Post Placement Coordinator.
Communicate with adoptive parent attorney in preparation for legal proceedings.
Attend adoptive family finalization hearings (Norristown, PA).
Support Biological Parent Case Manager as needed.
In person staff meetings (Wyndmoor or Ambler).
Complete required training hours (40 hours for full time) each year.
Collaborate with Director of Family Services and SWAN Supervisor for monthly Haven Happenings.
Organize/monitor adoption files for all required documents for annual inspection.
Participate in outreach for biological parents.
Collaborate with Director of Family Services to improve adoption education.

Pay: $22.00 to $26.00 per hour plus yearly bonus
