Job Overview
We are seeking a compassionate, dedicated, and highly skilled Registered Nurse (RN) to join our healthcare team.

In this role, the RN will monitor, coordinate, supervise, and evaluate healthcare services while ensuring compliance with Georgia Nurse Practice Act requirements, DBHDD regulations, Medicaid waiver standards, and each individual's Person-Centered Service Plan (PCSP). Responsibilities include conducting clinical assessments, developing and coordinating care plans, overseeing medication administration and management, providing staff training and education, and maintaining ongoing health surveillance.

The Registered Nurse will collaborate closely with physicians, direct support professionals, case managers, families, and other healthcare providers to ensure comprehensive, person-centered care. Strong clinical judgment, excellent communication skills, and the ability to work effectively within an interdisciplinary team are essential for success in this role.

Duties and Responsibilities

Conduct comprehensive patient assessments and develop individualized care plans tailored to meet each patient's healthcare needs.
Provide clinical oversight and supervision of Licensed Practical Nurses (LPNs), Certified Nursing Assistants (CNAs), and Direct Developmental Provider (DDP) staff to ensure quality care and compliance with established standards.
Monitor patient conditions, evaluate treatment effectiveness, and respond promptly to changes in health status.
Educate patients and their families on disease management, medication administration, preventive care, and healthy lifestyle practices.
Maintain accurate, timely, and compliant medical records, including assessments, and care plans.
Collaborate with physicians, therapists, social workers, case managers, and other interdisciplinary team members to coordinate comprehensive patient care.
Participate in Minimum Data Set (MDS) assessments and contribute to care planning and quality improvement initiatives.
Coordinate and support home and community-based care services to promote patient independence, safety, and well-being.
Ensure compliance with federal, state, and organizational healthcare regulations, policies, and procedures.
Advocate for patients' needs while promoting high standards of clinical care, patient safety, and positive health outcomes.

Qualifications

Current Registered Nurse (RN) license in the state of practice.
Minimum of two (2) years of nursing experience, preferably in home health or community-based care.
Knowledge of home care management, including patient assessment, care planning, and regulatory compliance.
Demonstrated ability to organize, coordinate, and supervise healthcare staff effectively.
Strong clinical judgment and decision-making skills in a fast-paced healthcare environment.
Excellent interpersonal and communication skills, with the ability to collaborate effectively within a multidisciplinary team.
Strong critical-thinking and problem-solving abilities, with a patient-centered approach to care.
Ability to prioritize multiple responsibilities while maintaining high standards of quality and patient care.
