TLC Nurse Solutions is seeking experienced independent Registered Nurses (RNs) to join an established private care team supporting an independent, active professional living with a high cervical spinal cord injury (C2 to C3, ASIA A) and ventilator dependence.

TLC Nurse Solutions is a California nurse registry. We coordinate scheduling, communication, and continuity of care by connecting clients with experienced independent nurses who provide services directly to private clients.

Schedule
Primary staffing needs include:
- Day Shift: 7:00 AM to 7:00 PM
- Night Shift: 7:00 PM to 7:00 AM
- Consistent long-term scheduling opportunities
- Typical availability ranges from approximately 24 to 48 hours per week based on client needs and nurse availability
- Optional travel assignments may become available and require independent travel arrangements
Clinical Responsibilities
- Ventilator and tracheostomy management, including airway clearance and emergency response
- Continuous assessment of respiratory status, skin integrity, positioning, and pressure injury prevention
- Proactive management of spinal cord injury related conditions including autonomic dysreflexia, orthostatic hypotension, and infection prevention
- Medication administration
- Bowel and bladder management
- Safely assist with transfers, positioning, mobility, and use of an electric lift
- Recognize changes in condition and communicate concerns promptly to support timely clinical decision-making
- Collaborate with physicians and other healthcare professionals regarding changes in the client’s condition as appropriate
Care Environment & Coordination
- Maintain a calm, respectful, and professional presence within a private home environment
- Support a structured, organized daily routine
- Maintain awareness of the client's care schedule and communicate scheduling updates or concerns that support continuity of care
- Collaborate professionally with the client, family, fellow independent nurses, and TLC Nurse Solutions to promote continuity of care
- Complete timely, accurate clinical documentation for each shift
- Help promote consistency and continuity across the care team
- Maintain a polished, professional appearance and demeanor that reflects the client's preference for a refined, client-facing environment
- Present professionally while accompanying the client to work, appointments, meetings, travel, or community activities

Required Qualifications
- Active California Registered Nurse license in good standing
- Tracheostomy experience required
- Minimum of 3 to 5 years of acute care or complex clinical experience (ICU, respiratory, neurology, trauma, or similar)
- Strong assessment and critical thinking skills
- Physically able to assist with transfers, repositioning, lift equipment, and emergency response
Preferred Experience
- Extensive experience caring for ventilator-dependent patients in an ICU, LTACH, or other high-acuity setting
- Private duty nursing
- Home healthcare
