Position Summary:

The Nurse Practitioner (NP) delivers high-quality, family-centered primary care services to pediatric and adolescent patients in a mobile healthcare setting. This role supports whole-student health by addressing physical, behavioral, and social determinants of health within school-based environments.

Program Overview:

Care Eleven Health’s Whole‑Person Primary Care model is a community‑driven, school‑integrated, team‑based primary care program that supports the health and wellness of the whole student and family. By weaving together clinical care, behavioral health, health education, and social support, all within the neighborhoods it serves, it delivers more accessible, equitable, and holistic care that moves beyond episodic visits to foster lifelong wellbeing

Key Responsibilities:

· Provide primary care services including well-child visits, physicals, immunizations, chronic disease management, and acute care for minor illnesses and injuries

· Conduct comprehensive health assessments and develop individualized care plans, ensuring alignment with best practices in family medicine

· Collaborate with mobile care team members (MAs, RNs, Care Coordinators) to support integrated, student-focused care

· Document all encounters and orders in the electronic health record (EHR) and ensure timely charting aligned with compliance and billing standards

· Participate in school-based care coordination efforts with educational staff, parents/guardians, and community resource partners

· Provide health education to students and families, ensuring clear understanding of diagnoses, treatments, and wellness plans

· Promote prevention and early intervention strategies across the student population, including screenings, vaccinations, and SDOH assessments

· Contribute to quality improvement efforts, ensuring care delivery meets regulatory standards and organizational expectations

· Maintain licensure and continuing education as required by state and organizational policy

Qualifications:

· Active and unrestricted Family Nurse Practitioner (FNP) license in the state of practice

· Board Certification as a Family Nurse Practitioner (AANP or ANCC)

· Minimum of one year of primary care or pediatric FNP experience preferred

· Experience working with school-aged populations, underserved communities, or in mobile/field-based settings is highly desirable

· Current CPR certification (BLS)

· Strong interpersonal and communication skills to engage children, adolescents, caregivers, and school staff

· Comfort working independently in a mobile or community-based setting

· Commitment to health equity, data-driven strategy, and community-rooted care.

· Knowledge of HIPAA privacy rules and regulations.

· Effective verbal and written communication skills and ability to present information clearly and professionally to varying levels of individuals (both patients and employees) throughout the patient care process.

· Ability to carry out detailed written or oral instructions.

· Ability to identify and resolve problems.

· Ability to establish and maintain effective working relationships with facility personnel, patients, and the public.

· Ability to exercise initiative, judgment, and decision-making in meeting department and institutional objectives.

Work Environment

· Duties are performed both in an office environment and in clinical settings; travel to clinics using employee's personal transportation is required.

· Because this position works within a healthcare setting, there may be occupational risk for exposure to hazardous medications or hazardous waste within the environment through receipt, transport, storage, preparation, dispensing, administration, cleaning and/or disposal of contaminated waste. The risk level of exposure may increase depending on the essential job duties of the role.

· Regular and predictable attendance is required for this full-time position. Workdays and hours coincide with clinic and/or office hours. Evening and weekend work may be required as job duties demand.

· This role routinely uses standard office equipment such as laptop computers, healthcare-specific devices, and various other digital devices.

Work Authorization

· Applicants must be currently authorized to work in the United States for any employer.

Work Location:

· Mobile health unit, servicing schools within the assigned region of Greater New Orleans and Baton Rouge area.
