Full Job Description

Midlothian Healthcare Center is seeking a compassionate, organized, and resident-centered Licensed Social Worker to join our leadership team. This position is ideal for a bachelor’s or master’s level social worker who enjoys building relationships with residents and families, supporting care transitions, and being part of a collaborative skilled nursing facility team.

The Social Worker plays a key role in promoting resident quality of life, psychosocial well-being, discharge planning, family communication, and regulatory compliance.

Responsibilities

The Licensed Social Worker will:

Complete psychosocial assessments, social history documentation, and ongoing resident interviews.
Support residents and families through the admission, transition, care planning, and discharge process.
Participate in interdisciplinary care plan meetings and ensure resident preferences, emotional needs, and social needs are addressed.
Assist with discharge planning, including coordinating community resources, home health, DME, hospice, outpatient services, and family education.
Advocate for resident rights, dignity, self-determination, and quality of life.
Communicate regularly with residents, families, nursing, therapy, admissions, business office, and outside providers.
Assist with grievance follow-up, concern resolution, and service recovery.
Support residents with adjustment to the facility, coping with illness, loss, behavioral health needs, or changes in condition.
Complete MDS/social services documentation timely and accurately.
Assist with advance directives, responsible party communication, and care conference coordination.
Maintain compliance with state, federal, and facility requirements related to social services in a nursing facility setting.
Help create a positive, welcoming, and supportive environment for residents, families, and staff.

Qualifications

Bachelor’s or Master’s degree in Social Work required.
Current Texas social work license required, such as LBSW, LMSW, or LCSW.
Experience in a skilled nursing facility, long-term care, hospital, hospice, home health, or healthcare setting preferred.
Strong communication, documentation, and problem-solving skills.
Ability to work collaboratively with an interdisciplinary team.
Must be compassionate, dependable, organized, and comfortable communicating with residents and families.
Knowledge of resident rights, discharge planning, care planning, and long-term care regulations preferred.
