Job Title: Registered Nurse
GENERAL SUMMARY OF DUTIES: The Registered Nurse (RN) in the Endoscopy Center is responsible for providing high-quality nursing care to patients throughout the pre-operative, intra-operative, and post-operative phases of their procedures. The RN ensures a safe environment for patients, upholds nursing best practices, and adheres to all policies and procedures. This role encompasses pre-operative preparation, intra-procedure sedation and monitoring, and post-operative recovery, discharge, and patient education.
ESSENTIAL FUNCTIONS:
1. Core Nursing Responsibilities:
- Treat all patients with dignity and respect
- Maintain confidentiality and privacy of all patients per HIPAA and Center policies.
- Adhere to GI Consultants Employee Code of Conduct, Core Customer Service requirements, departmental policies and procedures, and regulatory requirements.
- Function within the legal limits of nursing scope of practice and act as a patient advocate.
- Implement infection control measures and use personal protective equipment (PPE) as required.
- Ensure a safe environment for patients and staff, reporting any unsafe conditions or equipment concerns.
- Demonstrate knowledge of all equipment and supplies, troubleshooting as necessary.
- Work cooperatively and professionally with physicians and staff, following established protocols.
- Identifies patients using two identifiers at each point of care and communicates pertinent information when transferring patients from one care area to the next.
- Assess and monitor patients for abnormal physical symptoms, taking appropriate action.
- Administer medications and IV fluids appropriately based on patient age and condition.
- Demonstrate knowledge of crash cart contents, defibrillator, emergency medications, and appropriate response in emergency situations.
- Supervise and delegate tasks to Pre/Post-op Care Technicians to ensure smooth Center operations.
- Complete all electronic health records (EHR) prior to the end of the shift. Accurately document patient information and report errors of commission/omission.
- Communicate supply and medication needs to the Supervisor and/or Center Operations Director.
- Maintain continuous narcotic security according to policy.
- Ensure controlled medication count is correct at the beginning and end of shift.
- Document all controlled medications on the daily Controlled Substance Disposition Log.
- Waste all controlled medications with second RN verification and document accordingly.
- Participate in mock codes, fire drills, and annual competencies and trainings.
- Attend in-services and staff meetings as required
- All other duties as assigned
2. Pre-Operative Responsibilities:
- Verify procedure and sedation orders for accuracy and appropriateness.
- Review informed consent and sedation plan with the patient and/or guardian.
- Assess the patient's personal health information (PHI), including surgical history, medical conditions, and medication reconciliation.
- Evaluate NPO status, bowel preparation, fall risk, and mobility limitations.
- Identify allergies, sensitivities, and intolerances, documenting severity.
- Interpret cardiac rhythms, assess vital signs, and determine patient suitability for sedation.
- Initiate intravenous access using aseptic techniques and standard precautions.
- Provide pre-procedure education to patients regarding expectations and post-procedure care.
3. Intra-Operative Responsibilities:
- Ensure emergency medication, equipment, and suction are available and functional.
- Perform Sign In, Time Out, and Sign Out per the Safe Surgical Checklist.
- Administer light to moderate sedation as ordered and monitor patient response.
- Continuously assess vital signs, cardiac rhythm, and procedural tolerance.
- Identify and respond to sedation-related complications, ensuring patient safety.
- Employ techniques to maintain a patent airway as indicated and per scope of practice.
- Assist the anesthetist as a circulating RN during sedation with propofol.
- Support the physician in procedural tasks, documentation, and pathology collection.
- Maintain sterile technique as applicable (e.g., Liver Biopsy, PEG Placement).
4. Procedure-Specific Knowledge and Skills:
- Understand all procedures performed, interventions, and their potential complications (e.g., esophageal dilation or banding).
- Demonstrate correct use of and need for the Electro-Surgery Unit (ESU), including proper grounding pad placement.
- Understand different needs for patients with implantable devices such as magnet use during cautery and monitor settings.
- Identify anatomical landmarks in all endoscopy procedures.
- Understand basic principles and rationale for pathology collection in diagnosis and/or treatment of GI diseases (e.g., esophageal biopsies for Barrett's Esophagus).
- Assist the physician in recording and processing pathology specimens according to policy.
5. Post-Operative Responsibilities:
- Receive and review reports from the Procedure Room RN and Anesthetist.
- Assess and document post-sedation status, including Aldrete scores.
- Communicate abnormal assessments or concerns to the physician or anesthetist.
- Provide discharge instructions and education tailored to procedure findings.
- Perform medication reconciliation with patients and caregivers.
- Confirm follow-up appointments and ensure pathology specimen processing per policy.
- Identify and support fall risk patients, ensuring safe discharge criteria are met.
- Facilitate patient transport for higher levels of care if necessary.
EDUCATION:
- Graduate of an accredited nursing school program
- Bachelor of Science in Nursing (BSN) preferred
EXPERIENCE/SKILLS/KNOLEDGE:
- Administration and monitoring of moderate sedation (required)
- One (1) year of hospital nursing experience (required)
- One (1) year of experience in Gastroenterology/Endoscopy Nursing (preferred)
- One (1) year of Critical Care experience (preferred)
REQUIREMENTS:
- Current Nevada Registered Nurse (RN) license
- Advanced Cardiac Life Support (ACLS) certification (must obtain within one year of employment)
- Basic Life Support (BLS) for Healthcare Providers certification
