Part-Time Primary Care Physician (MD) – 3 Days Per Week

Location: Chicagoland Area (Skilled Nursing Facilities & Outpatient Clinics)
Job Type: Part-Time (3 Days Per Week)

Position Summary

Instavaxx Comprehensive Wound Care is seeking a compassionate and dedicated Primary Care Physician (MD) to join our growing team on a part-time basis (3 days per week). This position provides the opportunity to deliver high-quality primary care services to patients in partnering skilled nursing facilities, nursing homes, and outpatient clinic locations throughout the Chicagoland area.

As a Primary Care Physician, you will build long-term relationships with patients while managing acute and chronic medical conditions, promoting preventive care, and collaborating with an interdisciplinary healthcare team. Our flexible part-time schedule offers an excellent work-life balance while allowing you to make a meaningful impact on medically complex and geriatric patient populations.

ResponsibilitiesPrimary Care Services

Conduct routine primary care evaluations and follow-up visits.
Diagnose and manage acute and chronic medical conditions.
Perform annual wellness exams and preventive health assessments.
Develop and manage individualized treatment plans.
Prescribe and manage medications appropriately.
Coordinate patient care with specialists and other healthcare providers.
Monitor patient progress and adjust treatment plans as needed.
Provide compassionate, patient-centered care to long-term patients.

Facility & Clinical Responsibilities

Round at partnering skilled nursing facilities and nursing homes.
Provide care at designated outpatient clinic locations.
Maintain accurate, timely, and compliant medical documentation.
Collaborate with nurses, therapists, specialists, and administrative staff.
Ensure compliance with all federal, state, and facility regulations.
Participate in care planning to improve patient outcomes.

QualificationsRequired

Active Illinois MD license in good standing.
DEA registration and controlled substance license (if applicable).
Board Certified or Board Eligible in Family Medicine, Internal Medicine, Geriatrics, or a related specialty.
Excellent clinical judgment and diagnostic skills.
Strong communication and documentation abilities.
Ability to work independently while collaborating with interdisciplinary teams.
Reliable transportation and willingness to travel between facilities and clinic locations.

Preferred

Experience in primary care, skilled nursing facilities, long-term care, or post-acute care.
Experience caring for geriatric or medically complex patients.
Multiple years of clinical practice preferred.
Familiarity with electronic medical records (EMR).

Schedule

Part-Time
3 Days Per Week
