Full Job Description

Overview

Join our dedicated healthcare team as a Home Health Aide and make a meaningful difference in the lives of patients requiring personalized in-home care. In this vital role, you will provide compassionate assistance to individuals with diverse needs, promoting independence, comfort, and well-being. Your energetic and caring approach will help clients maintain their quality of life while ensuring their safety and dignity are prioritized at all times. This position offers an opportunity to work closely with healthcare professionals and develop your skills in a rewarding environment committed to excellence in patient care.

Duties

Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts when necessary
Monitor patient health by observing vital signs, managing medication administration, and reporting changes or concerns promptly to supervising healthcare providers
Support patients with memory care needs, including Alzheimer’s disease and dementia, by implementing specialized care techniques and ensuring a safe environment
Maintain accurate medical documentation within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems, ensuring compliance with home healthcare regulations and HIPAA confidentiality standards
Prepare nutritious meals tailored to individual dietary requirements while maintaining cleanliness through cleaning and laundry duties
Provide emotional support and effective communication to clients and their families, fostering trust and understanding during caregiving interactions
Assist with medical procedures such as catheter care, end-of-life care, and other complex patient personal care assistance tasks

Experience

Prior experience working in assisted living facilities, nursing homes, or home healthcare agencies is highly preferred
Demonstrated knowledge of medical terminology, patient transfer methods, and basic life support (BLS) including CPR certification
Experience working with geriatric patients, individuals with disabilities or developmental disabilities, dementia or Alzheimer’s care, and hospice or palliative care is advantageous
Familiarity with home health care assistance regulations and documentation practices using EHR or EMR systems
Proven ability to handle heavy lifting safely and perform physically demanding tasks such as using Hoyer lifts or assisting with mobility transfers
Strong communication skills for client interaction, family updates, and interdisciplinary team collaboration
Previous live-in employment experience is a plus but not required
