Full Job Description

In collaboration with San Francisco Family Treatment Court, we offer intensive substance dependency treatment for those looking to extend their recovery treatment beyond a residential setting.

POSITION OVERVIEW

The Case Manager is part of the Epiphany-FTC Project treatment team and is responsible for providing comprehensive case management services, formulation and implementation of the individual program plan, coordinating all services and referrals provided to clients and maintaining up to date data and written documentation.

Schedule:

8:30AM – 5 PM
Monday to Friday

QUALIFICATIONS

Bachelor's degree in behavioral science, preferred.
Certification as a drug and alcohol counselor or registered and in the process of certification.
Minimum 4 years of experience working with at-risk women, children, and families.
Possess valid CA Driver's License and a safe driving record for local travel only.
Must possess or be able to obtain current CPR and First-Aid certificates.

SKILLS AND KNOWLEDGE REQUIRED

Effective written and verbal communication skills.
Ability to work well within a multi-disciplinary team and follow trauma-informed principles.
Ability to collaborate effectively with families from diverse cultures.
Proficient in computer skills with various software as assigned.
Must meet agency conditions for employment including requirements for Child Abuse and Fingerprinting clearances.
Flexibility in working hours and days to accommodate program needs.

RESPONSIBILITIES

Participate in daily meetings to be updated on client needs and develop team plans.
Enter client data accurately into computer system as assigned.
Meet with clients weekly to provide case management and treatment planning.
Function as the Epiphany Center liaison with the Department of Social Services workers, Public Health, and other community agencies as needed.
Make referrals to other agencies and services as determined by the Individual Treatment Plan.
Facilitate Recovery and Life Skills groups as needed.
Demonstrate and maintain appropriate boundaries with clients and other staff.
Develop an after-care plan with each client and assist with transition into aftercare services.
Maintain client confidentiality as required by professional ethics and legal requirements.
Required to drive agency vehicles. Transport clients in agency vehicles as need to meet goals.
Communicate effectively with coworkers, clients and personnel from outside agencies.
Acknowledge safety policies and procedures and report potentially unsafe conditions.
Support and implement the Epiphany Center mission and values.
