Home Instead is seeking a Client Care Coordinator who combines a strong clinical background with consultative sales and relationship-building skills in order to help older adults and their families find the right care solutions.

This is an ideal opportunity for a healthcare professional who is compassionate, self-motivated and a problem solver. You will serve as a trusted resource for families, assess client needs, develop individualized care plans, train Care Professionals, build referral relationships, and help families take the next step toward receiving the support they need at home.

What You'll Do:

1. Client Acquisition & Care Consultations

Conduct Service Inquiries and in-person Care Consultations by doing a functional and home assessment using a consultative sales approach.
Develop tailored service plans that address each client’s individual needs.
Explain available care solutions and guide families through the decision-making process.
Maintain consistent communication and follow-up with prospective clients and families who have not yet committed to services.
Build and maintain relationships with clients, families, referral sources, and community care providers.

2. Client Onboarding and Care Professional Coordination

Coordinate client/Care Pro introductions for every new client and new Care Professional.
Partner with team members to coordinate the various aspects of a client’s care.
Ensure clients and families understand the services being provided and have the appropriate support as services begin.
Create a care plan and tasks to be used by Care Professionals in the home to ensure care is consistent and completed.

3. Ongoing Client Care & Quality Assurance

Maintain regular communication with clients and their families, Care Professionals, and other care partners to ensure quality service and client satisfaction.
Plan and execute a schedule that ensures each client receives at least one quality assurance check-in per month.
Continuously evaluate client needs and the effectiveness of the service plan.
Identify and implement appropriate service plan changes as client needs evolve.
Recognize opportunities to increase or adjust service hours and services.
Resolve client and family concerns to ensure needs are met and services remain appropriate.
Address questions or concerns and coordinate timely resolution of service issues.

4. Care Professional Training & Development

Support the onboarding and ongoing development of Care Professionals.
Participate in and/or facilitate Care Professional training, education, and meetings as assigned.
Reinforce company policies, procedures, care standards, and client-specific expectations.
Provide feedback and coaching related to client satisfaction, service quality, and Care Professional performance.
Identify training needs based on quality assurance visits, client feedback, and changes in client care needs.
Support a culture of professionalism, accountability, continuous learning, and high-quality client care.

What You Need:

Clinical Background - RN, LPN, PT preferred
3+ years of experience in the Senior Care or Health Care Industry.
The ability to work independently in a field position, with a self-starting, upbeat, positive, go-getter attitude.
Valid driver's license, clean driving record, and reliable vehicle for travel throughout Montgomery and Philadelphia Counties.
Excellent organizational skills, problem solving skills, and customer service.
Will bring their own team player attitude to our team-oriented environment.
A warm, friendly, compassionate demeanor with elderly clients.

Work Schedule: Monday through Friday 8:30 am - 4:30pm with limited on call responsibilities*. Flexibility to work one day per week remotely. Occasional weeknight or weeknights will be required with advance notice.

*Any on call work will be additionally compensation.
