Full Job Description

Social Services Director/Discharge Planner

Position Summary: The primary function shall serve as a resident advocate so as to ensure each resident's psychosocial, spiritual needs and wishes are met and maintained from the time of admission through discharge to ensure accurate rights and needs of each resident is met on a continuum.

Education and Experience Requirements:

Bachelor's degree in Social Work, or, Bachelors degree in a human service field including but not limited to sociology, special education, rehabilitation counseling, psychology, etc.
One year of supervised social work experience in a health care setting working directly with individuals.

Duties and Responsibilities including but not limited to:

Interviews prospective clients and presents all information pertinent to admission to the facility.

Makes all necessary arrangements with hospital staff, referring entities, etc., to ensure a timely and smooth admission process.

Completes all necessary admission paperwork with each resident and/or responsible party.

Competes all discharge planning making appropriate referrals upon discharge for the residents continuing care, equipment needs, home health, transportation and medications.

Participates on the Interdisciplinary Care Committee and completes the initial assessment MDS and care plan for each resident.
