Director of Connection Points / Memory Care Director

StoryPoint Kalamazoo

Position Summary:
The Director of Connection Points implements, trains, and facilitates all Memory Care specific initiatives within the community under the Connection Points Memory Care Approach.

With our approach’s unique initiatives, we can connect with residents in a more personal and meaningful way — enhancing the experience of our residents, families and team members. Desired Experience for Director of Connection Points:

Bachelor’s degree in Gerontology, Social Work, Human Services, Health Care Administration or related field preferred.
At least 2-year experience in Memory Care setting preferred.
Ability to manage multiple initiatives and complete assigned projects timely and accurately.
Ability to facilitate training programs.
Ability to build strong rapport with families, residents, leaders and team members.
Ability to self-audit Memory Care initiatives for effectiveness.
Proven organizational and communication skills.
Strong proficiency with Microsoft Office applications.

Accountabilities for Director of Connection Points:

Demonstrates and models company culture and values –safety, common beliefs, and a culture of feedback.
Demonstrated ability to be authentic, understands the value of devotion to employees and inspires excellence through empowering employees to pursue their best version.
Encourage, coach, and support Memory Care team members to inspire compassionate and positive relationships with residents.
Participate in hiring process for candidates who will be potentially working in Memory Care.
Model resident engagement presence when in neighborhood.
Complete or delegate 1440 neighborhood walk through and address issues as they are identified.
Responsible for communicating and completing a new resident move in plan with all appropriate team members/leaders.
Communicates to families of Memory Care residents often and participates in scheduled care conferences.
Formulate and sustain optimal relationships with residents and their families. Provide assistance and support as needed.
Ability to influence and optimize the efforts of others by relationship building and continuous coaching.
Implement and audit Memory Care training and programs.
Coordinate with community leadership team to ensure positive and meaningful experiences for Memory Care residents.
Attend weekly calls with Aging and Dementia Services.
Responsible for implementing, facilitating, and maintaining all Memory Care initiatives.
Facilitate a monthly family engagement event for family and caregivers.
Perform other duties as assigned.

General Working Conditions:
This position entails standing for long periods of time. While performing the duties of this job, the employee is required to communicate effectively with others, sit, stand, walk and use hands to handle keyboard, telephone, paper, files, and other equipment and objects. The employee is occasionally required to reach with hands and arms. This position requires the ability to review detailed documents and read computer screens. The employee will occasionally lift and/or move up to 25 pounds. The work environment requires appropriate interaction with others. The noise level in the work environment is moderate. Travel to different locations required.
