Full Job Description

Overview
We are seeking a dedicated and compassionate Care Navigator to join our healthcare team. The Care Navigator will play a crucial role in guiding patients through their healthcare journey, ensuring they receive the appropriate care and resources they need. This position requires you to have a care to visit patients homes in Brooklyn, Bronx, Long Island, and other nearby areas. Needs good communication skills, a solid understanding of medical platforms, and the ability to work collaboratively with healthcare nurses, patients, and their families.

Hours

Monday-Friday 9am-5pm

Responsibilities

Drive to patient's homes to assist patients in navigating the healthcare needs.
Complete necessary assessments and documentation as needed based on company protocol.
Educate patients and their care givers about their conditions, treatment options, and available resources to promote informed decision-making.
Monitor patient progress, follow up on care plans, and complete reassessments following company protocol.

Mandatory Skills

Bilungual (Spanish/English)
Proficiency in medical EMRs and understanding of case management principles.
Experience in care navigation and dementia-focused care.
Excellent communication skills, both verbal and written, with the ability to convey complex information clearly.
Ability to work effectively within a multidisciplinary team environment.
Strong organizational skills with attention to detail for accurate record keeping.

Job Type: Full-time

Pay: $25.00 - $30.23 per hour
