The Admissions Nurse is a registered nurse and an Interdisciplinary Team Member whose primary responsibility is to present RIVERCROSS services and philosophy to patients and families in a manner that result in the admission of appropriate patients.
Essential Job Functions/Responsibilities The Admission nurse will make an initial contact prior to the patient hospital discharge, if possible or appropriate or in the patient home/ facility, within 2 hours of receiving time frame. The purpose of the initial visit will be to:

Explain the hospice philosophy of palliative care with the patient and family/caregiver as unit of care.
Explain the patient's rights and responsibilities and grievance procedure
Provide the patient with a copy of Rivercross Healthcare notice of privacy practices.
Assess the family/caregiver’s ability to provide care.
Evaluate physical facilities and equipment in the patient's home to determine if they are safe and effective for care in the home.
Allow the patient and family/caregiver to ask questions and facilitate a decision for hospice services especially provided under the Medicare/Medicaid hospice benefit.
Review appropriate forms and subsequently sign forms by patient and family/caregiver once agreement for the hospice program has been decided.
Provide services as needed and ordered by physician (or other authorized independent practitioner), and incorporate additional needs into the hospice plan of care.
Give patient information about durable power of attorney for health care, if the patient has not already done so.
In instances where patient does not meet the stated criteria for admission to the program, exceptions will be decided upon by the Administrator in consultation with the Medical Director, upon request of the referring party and/or the patient.
The admitting registered nurse will explain the hospice election statement to the patient and family/caregiver or legal representative.
Responsible for starting and maintaining relationships with facilities, Physicians, and hospitals, in an effort to facilitate admissions
Assists in QAPI, and chart reviews to ensure compliance in all areas
Position also requires patient visits.
Keeps abreast of all compliance issues and eligibility requirements
Performs all other tasks as assigned by supervisor

Position Qualifications

Current RN license.
At least three (3) years’ experience in hospice or similar operation.
Complies with acceptable professional standards and practice.
Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.
Possess excellent critical thinking skills.
Team development skills to work as part of the team concept.
A passion for working with Hospice and helping patients and families to have a quality end of life experience.
Excellent clinical, end-of-life case management skills, with a strong background in comfort care.
Excellent observation skills, good nursing judgment, verbal and written communication skills, problem solving skills, basic math skills and nursing skills per competency checklist.
Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling and/or crouching. Visual acuity and hearing to perform required nursing skills.
Licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
Ability to drive at dawn or dusk.
