Patient Support Administrator at SyncHealth Management

About SyncHealth Management

To improve healthcare for patients experiencing Chronic illnesses and prevent hospital readmissions, between doctor visits. SyncHealth Management has an unorthodox approach that offers care coordination between the physician’s office and the patients home. The Patient Administrator will document and communicate with patients and assist the CCM Clinician with patient care plans.

Summary

Join SyncHealth Management as a Patient Support Administrator and play a crucial role in transforming healthcare for patients with chronic illnesses. By directly connecting the physician’s office with patients at home, you'll be at the forefront of our innovative care coordination approach. As a key member of our team, you will document patient interactions and support our CCM Clinician in developing personalized care plans. Responsibilities

● You will document patient interactions and updates accurately in our healthcare management system to ensure seamless communication between the clinical team and patients.

● We expect you to coordinate with CCM Clinicians to assist in developing, reviewing, and updating personalized care plans for patients with chronic illnesses.

● You shall connect with patients regularly to provide support, answer questions, and address concerns about their care plans, helping them adhere to medical advice.

● We expect you to facilitate effective communication between physicians' offices and patients by managing appointment schedules and reminders.

● You will conduct follow-up calls with patients after doctor visits or hospital discharges to reinforce care instructions and assess ongoing needs.

● You shall actively participate in team meetings and strategy sessions to contribute insights and feedback on improving patient care coordination processes.

● We expect you to maintain confidentiality of all patient information in compliance with HIPAA regulations and company policies.

● You will collaborate with multidisciplinary teams to streamline care coordination efforts and enhance overall patient satisfaction.

Requirements
