Full Job Description

Position Title: Community Coordinator - Homecare Department

Position Type: Full-time, Exempt

Reports To: Director of Homecare

Position Summary: The Community Coordinator is responsible for scheduling clients and caregivers to provide the highest quality service to clients with an emphasis on creating extraordinary relationships. The Community Coordinator collaborates with the Homecare Department, Nurse Department, and Managed Care Organizations (MCOs) to ensure clients receive the best possible care in their homes and community.

Duties and Responsibilities:

Promote home care services, vocational training, and various social services to individuals, local businesses, faith and community-based organizations.
Interact with participants and home health aides via telephone, email or in-person to provide excellent customer service and support for home healthcare services and needs.
Be able to multi-task between scheduling, recruiting and onboarding new home health aides.
Create and maintain client file and home health aide records.
Enter scheduling data, detailed notes and document issues and/or concerns into a scheduling software program.
Monitors and addresses Electronic Visit Verification (EVV) and clocked timesheets for caregivers to ensure payroll is processed timely.
Serve as a liaison between case managers, Area Agency on Aging (AAA) officials, Managed Care Organizations (MCOs), home health aides, and home care clients (and their families as needed) to ensure seamless delivery of services.
Assist management in the development of linguistically and culturally appropriate outreach strategies.
Develop bilingual outreach materials.
Provide interpretation as required at home health aide certification sessions, in-service trainings, intake assessments, etc.
Attend and contribute to agency staff meetings and represent agency at community events as needed.
Other related duties as necessary.

Qualifications:

US college degree preferred.
One year community outreach and/or health education experience preferred. Demonstrated knowledge of Vietnamese communities in Philadelphia and surrounding areas.
Bilingual in Vietnamese required.
Ability to build trust quickly with community members and agency partners.
Excellent customer service skills.
Ability to work as part of a team or independently as duties require.
Demonstrated ability in setting and achieving goals within a given timeframe.
Strong communication skills, both verbal and written.
Willingness to work some evenings and weekends; local travel required.
Knowledge and ability to accurately enter data into computer software.

Job Type: Full-time

Pay: $21.00 - $25.00 per hour
