Deliver Care Where It Matters Most
You'll provide comprehensive primary care throughout assisted living, memory care, skilled nursing, and senior housing communities across Southern Minnesota.
Rather than spending your entire day inside one clinic, you'll work directly within the communities you serve, caring for patients with complex medical needs while collaborating closely with nurses, therapists, caregivers, social workers, and facility leadership.
You'll have the opportunity to know your patients, understand their histories, recognize changes in condition, and provide proactive care before smaller concerns become larger medical problems.
Your work helps reduce unnecessary hospitalizations, strengthen continuity of care, and keep seniors healthier, safer, and more independent.
What You'll Be Doing
- Perform comprehensive patient assessments and routine provider visits.
- Diagnose and manage acute and chronic medical conditions.
- Develop individualized treatment plans based on each patient's clinical needs and goals.
- Provide preventative care and ongoing chronic disease management.
- Coordinate care following hospitalizations, rehabilitation stays, and specialist visits.
- Collaborate with physicians and interdisciplinary healthcare teams.
- Communicate with nursing staff, patients, families, caregivers, and senior living leadership.
- Educate patients, families, and caregivers regarding diagnoses, medications, and treatment plans.
- Participate in quality improvement initiatives.
- Help improve patient outcomes through proactive and preventative medicine.
- Maintain accurate and timely clinical documentation.

What We're Looking For
Required Qualifications
- Active Minnesota Nurse Practitioner license.
- Excellent assessment, diagnostic, and clinical decision-making skills.
- Ability to practice independently while collaborating with physicians.
- Strong communication and care coordination skills.
- Passion for serving older adults.
- Reliable transportation and willingness to travel throughout Owatonna, Rochester, Albert Lea, and surrounding Southern Minnesota communities.
Preferred Qualifications
- Advanced wound care experience.
- Chronic disease management expertise.
- Community-based or senior care experience.
- Experience caring for medically complex older adults.
- Wound Care Certification (WCC) is a plus.
