Job Overview
We are seeking a dedicated and compassionate Case Manager to join our dynamic team. In this vital role, you will serve as a guiding force for individuals navigating complex social and health challenges. Your proactive approach will empower clients to access essential services, develop personalized care plans, and achieve their goals. This position offers an exciting opportunity to make a meaningful impact in people's lives by providing comprehensive support, crisis intervention, and resource coordination. Your energy, empathy, and organizational skills will drive positive change within our community.

Responsibilities

Conduct thorough assessments to identify clients’ needs across behavioral health, social work, child welfare, and workforce development domains
Develop and implement individualized care plans that align with clients’ goals and circumstances
Provide crisis intervention and crisis management support during urgent situations to ensure safety and stability
Offer addiction counseling and behavioral health guidance to promote recovery and well-being
Utilize motivational interviewing techniques to encourage positive behavioral changes and enhance client engagement
Coordinate with external agencies such as Section 8 housing programs, Medicare services, and child welfare organizations to facilitate access to resources
Supervise and mentor junior staff or interns, fostering professional growth and ensuring quality service delivery
Maintain detailed documentation of client interactions, progress notes, and service plans in compliance with organizational policies
Advocate for clients’ needs by connecting them with childcare services, mental health resources, or workforce development programs as appropriate

Experience

Proven experience in case management within social work, behavioral health, or HIV related fields
Strong background in crisis intervention and crisis management techniques
Hands-on experience with HIV treatment,
Familiarity with social service systems
Demonstrated supervisory or mentoring experience with team members or interns
Knowledge of child welfare practices and childcare considerations in case planning
Excellent communication skills with the ability to motivate clients through motivational interviewing approaches
Experience with Insurance Programs/Enrollment
Experience with eCW preferred
Ryan White experience desired
500/501 Certification
