Full Job Description

Full-Time LVN – Kaiser Case Manager

We are seeking a dedicated and experienced Licensed Vocational Nurse (LVN) to join our team as a Kaiser Case Manager. This full-time position is responsible for coordinating patient care, communicating with healthcare providers, supporting members throughout the care process, and ensuring timely follow-up and appropriate utilization of healthcare services.

The ideal candidate is organized, compassionate, detail-oriented, and comfortable working independently while collaborating with Kaiser Permanente healthcare teams and other care partners.

Key Responsibilities

Coordinate and monitor patient care from initial referral through follow-up and resolution.
Communicate with Kaiser Permanente physicians, nurses, patients, families, and other healthcare professionals.
Review patient information, medical records, treatment plans, and care needs.
Assist with coordinating appointments, referrals, authorizations, and follow-up services.
Identify barriers to care and assist patients in accessing appropriate healthcare resources.
Conduct timely outreach to patients and provide education regarding care plans and available services.
Track patient progress and document all care coordination activities accurately.
Collaborate with interdisciplinary teams to promote continuity, quality, and cost-effective care.
Identify changes in patient status or care needs and escalate concerns appropriately.
Maintain confidentiality and comply with HIPAA and applicable healthcare regulations.
Complete required documentation and case management activities within established timeframes.
Perform other duties related to care coordination and case management as assigned.

Qualifications

Current and active LVN/LPN license
Prior healthcare, clinical, utilization management, care coordination, or case management experience preferred.
Experience working with Kaiser Permanente or other managed-care organizations is a plus.
Strong knowledge of clinical terminology and healthcare processes.
Excellent verbal and written communication skills.
Strong organizational and time-management abilities.
Proficiency with electronic medical records and computer systems.
Ability to work independently, prioritize multiple cases, and meet deadlines.
Professional, compassionate, and patient-centered approach.

Pay: From $85,000.00 per year
