Full Job Description

Position Summary

The Designated Coordinator is responsible for coordinating, monitoring, and evaluating services provided to individuals receiving support under Minnesota Statutes Chapter 245D. This position ensures services are person-centered, compliant with state and federal regulations, and designed to help individuals achieve their identified goals and outcomes.

Essential Duties and Responsibilities

● Coordinate and oversee implementation of each person’s Coordinated Service and Support Plan (CSSP) and applicable addendums.

● Monitor service delivery to ensure supports are provided according to the individual’s assessed needs and desired outcomes.

● Supervise, Direct Support Professionals implementing service plans.

● Conduct direct observations of staff performance and assess competency as required under 245D.

● Develop, review, and update individual service plans in collaboration with the person served, legal representatives, case managers, and interdisciplinary team members.

● Monitor progress toward individual goals using measurable outcomes and make recommendations for service modifications when needed.

● Maintain accurate, timely, and compliant documentation, including progress notes, assessments, incident reports, and required records.

● Participate in person-centered planning meetings and communicate regularly with families, guardians, case managers, and community partners.

● Ensure health, safety, rights, and dignity of individuals receiving services.

● Assist with investigations, incident follow-up, corrective actions, and quality assurance activities.

● Provide training, mentoring, and ongoing support to staff regarding company policies, person-centered practices, and 245D requirements.

● Support compliance with licensing standards, DHS regulations, and agency policies.

● Participate in on-call responsibilities as assigned.

Qualifications

Candidates must meet Minnesota 245D Designated Coordinator qualifications, including education, training, and experience requirements established under Minnesota Statutes §245D.081. Qualifications generally include a combination of education in human services, education, or health-related fields and relevant experience providing direct support services to individuals with disabilities or older adults.

Knowledge, Skills, and Abilities

● Strong understanding of Minnesota 245D licensing requirements.

● Knowledge of person-centered planning and positive support strategies.

● Excellent leadership, communication, and organizational skills.

● Ability to supervise staff and promote quality service delivery.

● Strong documentation and computer skills.

● Ability to maintain confidentiality and professional boundaries.

● Effective problem-solving and conflict-resolution abilities.

Physical Requirements

● Ability to assist individuals with mobility and daily living activities.

● Ability to sit, stand, walk, bend, and drive for extended periods.

Working Conditions

Work is performed in residential and community settings. may be required based on program needs.
