Full Job Description

About the Position:

As a frontline representative of the organization, this Independent Living Specialist (ILS) serves as an initial point of contact, engaging with individuals with disabilities, their families, and the broader community through in-person, phone, and email interactions to connect them with appropriate disability-related resources, services, and community partnerships. These efforts extend to other related areas that contribute to achieving, sustaining, and enhancing community-based living and independence. The primary role of the Independent Living Specialist is to support housing navigation and efforts to sustain housing. This may include providing Independent Living Skills (ILS) training on tasks related to community-based living, such as job navigation, organization, or accessing food pantries. This position provides services in person and is an on-site role.

Essential Functions

Provide services in alignment with the Independent Living Philosophy and ACC's practices and service standards.
Conduct outreach to community partners such as One Stop, Homeless Services, Housing Authorities, and other community based organizations
Support housing navigation, including assistance with housing search, rental assistance applications, landlord/property manager communication, recertification, and education on tenant rights and responsibilities.
Connect clients to personal care supports through IHSS, private pay providers, ACC's QuickMatch registry, and/or Bridging the Gap; assist with PA recruitment and interviews as needed.
When appropriate, refer clients through CalAIM Community Supports for housing-related services in accordance with all CalAIM requirements.
Meet monthly caseload minimums including CalAIM requirements.
Ensure clients attain and maintain benefits to support housing; collaborate with ACC's Benefits Specialist as needed.
Provide ILS training in areas including self-advocacy, housing benefits, transportation, in-home care, and community-based living.
Maintain current knowledge of local programs, resources, and organizations supporting independent living, housing, and disability services; make internal and external referrals using a "no wrong door" approach.
Represent ACC at community events and serve on committees to promote systems advocacy in housing, transportation, and personal assistance.
Maintain accurate, timely client records in CIL Suite; compile and submit service data as required.
Provide Spanish language support and translation for Spanish-speaking clients as needed.
Attend staff meetings, supervision, and training; follow all ACC policies, procedures, and service standards.
Other duties as assigned.

Essential Qualifications

Familiarity with human services programs supporting people with disabilities of all ages
Strong interest in disability rights and independent living
Excellent written, verbal, and interpersonal communication skills
Ability to deliver training and implement varied service models
Strong organizational skills and attention to detail; able to meet deadlines
Experience with service documentation and record-keeping
Proficient in computer use, including data entry, research, and social media
Reliable personal vehicle for work-related travel
Valid driver’s license and satisfactory driving record
Ethical, dependable, and team-oriented work approach

Preferred Qualifications

Strongly preferred: Personal experience with disability.
Bilingual English/Spanish or English/ASL.
Experience with CalAIM

Work Environment/Physical Demands:

Work in the office and in the community, including office buildings and clients’ place of residence (home, apartment, facility, hospital, etc). Normal office-temperature controlled conditions with quiet to moderate noise level and little to no exposure to heat, dust, or fumes. While performing duties of the job, employee is regularly required to remain in a stationary position working at a desk, operating a computer, and other office equipment. May be required to lift or move up to 10lbs.
