Youth Service Bureau (YSB) is dedicated to ensuring all youth have access to safety, shelter, and opportunities for success. We provide a range of services, including transitional living programs for pregnant and parenting young adults, as well as non-parenting young adults ages 18–21.
Our goal is to help young people rebuild their lives after experiences of abuse, neglect, homelessness, housing instability, and lack of resources by providing safety, support, and opportunities to build independent and successful futures.

Youth Service Bureau delivers client-centered, trauma-responsive, and preventative services and support to youth experiencing safety and housing crises.
Position Summary
The FYSB MGH & TLP Case Manager is a dual-program position providing case management services within YSB's Beating the Odds (BTO) and Porch Light (PL) Transitional Living Programs.
The Case Manager will work with both parenting and non-parenting young adults ages 18–21, providing individualized and intensive case management using trauma-informed care, Positive Youth Development, strengths-based practices, and evidence-based approaches.
This position is responsible for maintaining a caseload across both programs and assisting clients with developing and achieving individualized goals related to housing, education, employment, financial stability, health and wellness, parenting, permanent connections, and independent living.
The Case Manager will provide consistent case management sessions, coordinate services and referrals, facilitate life skills programming, provide aftercare services to clients who have exited the program, and assist with resident activities and outings.
The Case Manager will maintain a client-centered, solution-focused approach while supporting young adults in building the skills and resources necessary for long-term independence and stability.
What You'll Do
Client Engagement & Assessment
- Conduct thorough, trauma-informed intakes and assessments with parenting and non-parenting young adults to identify strengths, needs, goals, barriers, and potential risk factors.
- Develop individualized service plans in partnership with clients that reflect goals related to housing stability, education, employment, financial stability, health, parenting, independent living, and social support.
- Build professional, supportive relationships with clients while maintaining appropriate boundaries.
- Utilize trauma-informed care, Positive Youth Development, and strengths-based approaches in all client interactions.
Case Management & Service Planning
- Provide consistent, individualized case management to assigned clients across both BTO and Porch Light.
- Conduct regularly scheduled case management sessions and maintain ongoing communication with assigned clients.
- Create, maintain, and update individualized service and case plans that identify goals, interventions, timelines, action steps, and measurable outcomes.
- Regularly assess client progress and follow up on established goals and action steps.
- Assist clients in identifying barriers to progress and developing realistic strategies to overcome those barriers.
- Coordinate access to community services, including physical and mental health care, substance use services, education programs, employment resources, parenting support, childcare, and other appropriate services.
- Facilitate connections to mainstream benefits and community resources and follow up to ensure referrals are completed.
- Respond appropriately to client needs, emergencies, and crises.
Housing & Safe Exits
- Work directly with clients to identify safe, appropriate, and stable housing options throughout program participation and in preparation for program exit.
- Assist clients with housing searches, applications, communication with landlords and housing providers, and connection to housing assistance programs.
- Provide coaching on independent living skills, including budgeting, money management, tenant responsibilities, household management, and other daily living competencies.
- Support clients in transitioning from BTO and Porch Light into safe and stable housing, including independent living, supportive housing, family reunification when appropriate, and housing assistance programs.
- Develop safe and appropriate exit plans with clients and coordinate resources needed to support long-term housing stability.
- Provide aftercare services and follow-up to clients who have successfully exited the program.
Education & Economic Stability
- Assist clients with high school enrollment, diploma or GED/HSE attainment, vocational training, college enrollment, and career planning.
- Provide employment readiness assistance, including job searches, applications, resumes, interview preparation, and connections to workforce development resources.
- Support clients in identifying realistic educational and career goals and developing action steps toward achieving them.
- Monitor and document client participation and progress toward educational, employment, and financial goals.
Parenting & Child Development Services
- Provide or coordinate parenting education and life skills programming that support the social-emotional development and overall well-being of young parents and their children.
- Assist parenting clients with developing skills related to child health and safety, attachment, developmental milestones, routines, and positive parenting.
- Connect clients to prenatal and postnatal care, early childhood education, childcare, developmental resources, and other family support.
- Support parenting clients in balancing parenting responsibilities with education, employment, and independent living goals.
Life Skills & Client Programming
- Plan, coordinate, and facilitate life skills programming based on client needs and program goals.
- Provide education and coaching related to budgeting, financial literacy, household management, cooking, transportation, employment, education, communication, healthy relationships, self-advocacy, and other independent living skills.
- Plan and facilitate social activities, events, and outings for BTO and Porch Light clients.
- Encourage client participation in activities that promote healthy relationships, community connections, personal development, and independence.
Client Transportation & Vehicle Use
- Provide safe and reliable transportation for clients to appointments, employment and education activities, community resources, housing-related needs, and program outings.
- Operate agency vehicles as part of job duties, including a 16-passenger van, in accordance with all safety policies and training requirements.
- Ensure proper supervision and safety of clients while transporting them in agency vehicles.
- Follow all agency transportation procedures, including vehicle inspections, safety checks, and documentation requirements.
- Support community-based engagement by transporting clients to off-site services, activities, and appointments as needed.
Documentation & Data Reporting
- Maintain accurate, thorough, and timely documentation in HMIS and other required agency or grant reporting systems.
- Complete case notes, service plans, assessments, unit checks, client interactions, incident reports, referrals, and communication logs within required agency timeframes.
- Maintain organized and up-to-date client records in accordance with agency policies and grant requirements.
- Monitor and document client progress toward required performance outcomes, including housing, employment, education, health, and independent living goals.
- Compile and submit performance data as requested to support compliance with funding requirements and grant reporting timelines.
Trauma-Informed & Culturally Responsive Practice
- Deliver services using a trauma-informed, strength-based, and client-centered approach that respects cultural, linguistic, developmental, and individual differences.
- Recognize and appropriately respond to signs of trauma, human trafficking, exploitation, abuse, neglect, and other complex issues affecting clients and their children.
- Maintain professional and ethical boundaries while creating an environment where clients feel respected, supported, and empowered.
- Promote client choice and self-determination while maintaining appropriate program expectations and safety standards.
Advocacy & Client Support
- Advocate on behalf of clients with service providers, schools, landlords, employers, healthcare providers, and social service agencies.
- Assist clients in developing self-advocacy skills and increasing their ability to independently navigate community systems.
- Provide crisis intervention and support during emergencies.
- Participate in safety planning and connect clients to medical, mental health, legal, or other emergency services when necessary.
Collaboration & Community Engagement
- Build and maintain effective partnerships with community service providers, healthcare agencies, housing organizations, educational institutions, workforce programs, and other organizations that strengthen resources available to BTO and Porch Light clients.
- Participate in community meetings and interagency collaborations as appropriate to enhance referral networks and program visibility.
- Maintain effective communication with residential staff, therapists, supervisors, and other YSB team members to ensure continuity of care.
- Coordinate services across programs and community providers to reduce barriers and support client success.
Facilities & Environment of Care
- Conduct routine environment-of-care, apartment/unit, and safety checks to ensure client living spaces are safe, sanitary, and compliant with agency and FYSB requirements.
- Identify, document, and report facility and safety concerns.
- Submit maintenance requests as needed and escalate urgent concerns to ensure timely resolution.
- Collaborate with residential staff to maintain a trauma-informed, child-safe, clean, and welcoming environment.
- Reinforce expectations related to cleanliness, shared spaces, client responsibilities, and program safety.
Quality Assurance & Professional Development
- Maintain compliance with FYSB grant requirements, agency policies, program procedures, confidentiality standards, and applicable regulations.
- Attend required agency and program staff meetings.
- Participate in ongoing training related to trauma-informed care, Positive Youth Development, human trafficking, crisis intervention, youth development, housing instability, and other relevant best practices.
- Participate in regular supervision with the TLP Supervisor to review client needs, discuss cases, monitor performance, address barriers, and support continued professional development.
- Participate in program evaluation and quality improvement efforts as requested.
Requirements
Education: Bachelor's degree preferred in Social Work, Psychology, Human Services, Criminal Justice, Early Childhood Education, or related field.
Experience: Two years of social work, case management, residential services, youth services, or related human services experience preferred.
Technology Skills: Basic computer proficiency, including Microsoft Office 365. Ability to learn and consistently utilize HMIS and other required data systems.
Communication Skills: Must possess strong written and verbal communication skills and be able to communicate professionally with clients, coworkers, community partners, and service providers.
Independent Work & Time Management: Must be able to work independently, manage competing priorities, meet documentation deadlines, maintain an assigned caseload, and seek guidance when appropriate.
Trauma-Informed Practice: Must be able to integrate trauma-informed care, Positive Youth Development, strengths-based practices, and transition-to-independence skills into client interactions while maintaining ethical and professional boundaries.

Other Important Information
Reports To: TLP Supervisor
Typical Work Week: Monday–Thursday, 11:00 a.m.–7:00 p.m., plus alternating Saturdays and Sundays, 11:00 a.m.–7:00 p.m. This is an in-person position.
Travel: This is a dual-program position requiring travel between YSB program locations and off-site client apartments for case management appointments, unit checks, client needs, community-based services, trainings, and other work-related responsibilities. Primary work locations are within South Bend and Mishawaka.
