Weekend Clinical Manager

(Remote Work Environment)

Position Summary

The Weekend Clinical Manager is responsible for ensuring the delivery of high-quality, compliant patient care services during weekend operations. This role provides clinical leadership, oversees scheduling functions, supports field clinicians, and manages time-sensitive documentation workflows. The Weekend Clinical Manager serves as the primary on-call resource for patients, caregivers, and staff, ensuring continuity of care and timely resolution of clinical and operational issues.

The position collaborates closely with intake, clinical field staff, physicians, and leadership to support safe, effective, and efficient home health operations during weekends and holiday coverage periods.

Essential Duties and Responsibilities

Clinical Oversight & Patient Care Coordination

Serve as a clinical resource for field clinicians by providing guidance on patient care, regulatory compliance, documentation standards, and best practices.
Support clinical decision-making and problem resolution in real time.
Review admission, recertification, Start of Care (SOC), and physician order documentation to ensure completeness, accuracy, and compliance with agencyand regulatory standards.
Ensure the completion of individualized Plans of Treatment in accordance with physician orders, patient needs, insurance guidelines, and agency protocol.
Coordinate with physicians, nurses, discharge planners, and referral sources regarding patient care needs and continuity of services.
Ensure appropriate DME, medical supplies, and ancillary services are ordered and documented as needed.
Monitor patient care delivery to ensure adherence to agency policies, procedures, and applicable federal and state regulations.

Weekend Operations & On-Call Management

Act as the primary on-call contact for patient, caregiver, and staff inquiries durin weekend hours.
Triage and respond promptly to urgent clinical and operational issues.
Escalate significant patient care, staffing, or compliance concerns to leadership as appropriate.
Maintain continuity of operations and facilitate effective communication between weekend and weekday teams.
Document key events, follow-up items, and unresolved issues for leadership handoff.

Scheduling & Staffing Oversight

Manage and adjust clinician schedules to ensure adequate weekend coverage and productive assignments.
Address last-minute call-outs, staffing gaps, and urgent patient care needs.
Coordinate scheduling workflows to support Monday staffing readiness and continuity of patient care services.
Delegate tasks to support staff and clinicians to ensure timely and efficient operations.

Documentation & Compliance Management

Perform first-level review of clinical documentation, including SOC and recertification records.
Monitor documentation submission timelines and follow up on incomplete or late documentation.
Facilitate timely submission and approval of clinical documentation to support billing and regulatory requirements.
Demonstrate working knowledge of OASIS requirements, Medicare/Medicaid
regulations, reimbursement practices, and home health documentation standards.
5. Participate in quality improvement and compliance activities as assigned.

Communication & Leadership

Serve as a liaison between field staff, intake, patients, caregivers, and leadership during weekend operations.
Maintain professional, respectful, and culturally sensitive communication with patients, families, referral sources, and staff.
Assist with orientation and support of new clinical personnel as needed.
Participate in meetings, case conferences, and handoff communications as required.
Demonstrate leadership, sound judgment, initiative, and effective problem-solving skills in a fast-paced environment.

Qualifications

Education & Licensure

Current, active Registered Nurse (RN) license in good standing in the applicable state.
BSN preferred.

Experience

Minimum of five (5) years of nursing experience required.
Minimum of two (3) years of home health (CHHA) experience preferred.
Prior supervisory, charge nurse, or clinical management experience preferred.

Knowledge, Skills & Abilities

Strong clinical judgment and decision-making skills.
Knowledge of home health regulations, OASIS, and clinical documentation standards.
Excellent organizational and multitasking abilities.
Ability to manage multiple priorities in a fast-paced environment with minimal supervision.
Strong interpersonal, communication, and customer service skills.
Proficiency with EMR systems, scheduling platforms, and Microsoft Office applications.
Ability to maintain confidentiality and professionalism at all times.

Work Schedule

Weekend and Holidays coverage schedule (Saturday and Sunday and legal holidays).
On-call availability during assigned shifts and holiday rotations as required.
Hybrid work environment with remote responsibilities and occasional in-office or field support as needed.

Physical Demands & Work Environment

Frequent sitting, computer work, and phone communication.
Occasional standing, walking, bending, and travel as needed.
Ability to work under frequent interruptions and changing priorities.
Exposure to clinical and home care environments as applicable.

Compliance Statement

The Weekend Clinical Manager is expected to comply with all agency policies and procedures, federal and state regulations, HIPAA requirements, and organizational compliance standards. All other duties may be assigned as necessary to support agency operations and patient care services.

Pay: From $62.50 per hour

Work Location: Hybrid remote in Flushing, NY
